Immunization and Vaccines for Children Program - 2025-2030 Cooperative Agreement Abstract South Carolina (SC) is applying for the 2025 Cooperative Agreement (CoAg) which is part of the 2025-2030 Project Period. SC will utilize the funding awarded to meet several activities to increase immunization rates. In doing so, SC will address all strategies of the CoAg’s three components [Core, Rapid small-scale response vaccine preventable disease (VPDs), and Rapid large-scale response to VPDs] through the following items throughout the project period: • Strengthen Program Infrastructure and Management: o Under CDC guidance, SC, will implement immunization programmatic activities which will benefit state, regional and local health jurisdictions while monitoring budgets and all funding to ensure expenditures are appropriate by funding source. o SC will perform individual provider coverage rate assessments utilizing the tool in the IIS. o Adult Immunization, SC will collaborate with providers and partners to increase adult vaccination coverage by promoting quality improvement activities, awareness and education targeting parents, providers and adults. • Increase Vaccine Access: o SC will enroll immunization providers through the IIS who will administer vaccines in compliance with CDC vaccine guidance. The providers will participate in quality assurance visits and ensure proper storage and handling of vaccines. o Perinatal Hepatitis B Prevention, SC will identify HBs-Ag-positive pregnant women and provide case management for the mother and infant to ensure completion of prophylaxis and post vaccination serology. • Improve Vaccination Equity: o SC will identify populations with low coverage and implement activities to improve coverage utilizing the Immunization Information System (IIS) to determine areas in the state and prioritize areas with low coverage areas of focus. SC will integrate NIS. • Promote Vaccine Confidence and Demand: o SC will collaborate with stakeholders to promote and educate the public and providers to increase immunization coverage. • Enchance Data and Evaluation: o SC IIS will be utilized by all immunization providers statewide as mandated by state law. o SC will explore establishing protocols to share IIS data with to integrate NIS. o SC will o Surveillance: SC will have established surveillance policies and procedures to address vaccine preventable diseases. • Strengthen Program Support for Partners: o School Vaccination Coverage: SC will assess vaccination coverage and exemption levels for public and private schools and will annually report data to CDC. SC shares summary data with the South Carolina Department of Education. o Adolescent Immunization: SC will collaborate with providers and partners to increase adolescent vaccination coverage by promoting quality improvement activities, awareness and education targeting parents, providers and adolescents. • Enhance Vaccination Response Readiness: o Emergency Response: SC will have established emergency response policies and procedures to mobilize for rapid small and rapid large scale VPDs public health emergencies. o Outbreak Response: SC will have established outbreak policies and procedures to address vaccine preventable diseases. o Pandemic Influenza Preparedness: In conjunction with preparedness program activities, SC will maintain a listing of vaccine providers and assess the capacity of state and local resources to address SC’s response to pandemic influenza.
