The Arizona Association of Community Health Centers (AACHC) will continue its Connecting Arizonans to Coverage (CA2C) project to provide Navigator assistance for consumer access to healthcare. - The Arizona Association of Community Health Centers (AACHC) is the Primary Care Association for Arizona. We exist to support and grow the healthcare safety net that includes Community Health Centers (CHCs), Look-alikes, and Federally Qualified Health Centers (FQHCs).Through this new Connecting Arizonans to Coverage (CA2C) project period, AACHC will continue our work of the past 11 years as a consortium grantee. Our 13 subrecipients include the following Community Health Centers, community based, and community-focused organizations: 1. Adelante Healthcare, Inc. 2. Asian Pacific Community in Action (APCA) 3. Chiricahua Community Health Centers, Inc. 4. Chicanos por la Causa (CPLC) 5. El Rio Santa Cruz Neighborhood Health Center, Inc. 6. Helping Families In Need (HFIN) 7. Mariposa Community Health Center, Inc. 8. North Country HealthCare (NCHC) 9. Neighborhood Outreach Access to Health (NOAH) 10. Sun Life Health 11. Sunset Health 12. Tuba City Regional Health Care Corporation 13. University of Arizona Center for Rural Health (AzCRH) AACHC and its subrecipients will serve all 15 counties in the State of Arizona. We are requesting $3,500,000 for Year 1; $3,500,000 for Year 2; $3,500,000 for Year 3; $3,500,000 for Year 4; and $3,500,000 for Year 5, for a total of $17,500,000. The CA2C project will target all priority populations including racial & ethnic minorities, rural communities, LGBTQ+, low income or unhoused individuals, American Indian or Alaska Natives, people with physical or intellectual disabilities, individuals with cognitive, hearing, speech and/or vision impairments, pregnant women, new mother, women with children, individuals with mental health or substance-related disorders, individuals with HIV/AIDS, Medicaid eligible consumers who are not enrolled in coverage despite being eligible for Medicaid. AACHC and its subrecipients will have 85 Navigator staff trained and certified or recertified by October 2024. At least 5 million impressions will occur through an online and statewide education and outreach campaign, 75 presentations, 750 events, and 5,500 consumer materials. At least 15,000 appointments will be scheduled on the Cover Arizona Connector; 2,400 consumers will be assisted with enrollment or re-enrollment in a QHP; 100,000 interactions will occur between Navigators and consumers; 12,000 consumers will be assisted with understanding the basic concepts and rights related to health coverage etc.; 22,500 will be assisted with Medicaid/CHIP applications or referrals; 140 will be assisted with Marketplace forms and exemptions, etc.; 200 with complex case referrals, data matching issues, etc.; 2,750 will receive referrals to other entities for complaints, questions and grievances, etc.; 5,000 will receive referrals to community resources; and 8,500 will receive assistance on how to use their health coverage.
