Cooperative Agreement to Support Navigators in Federally-facilitated and State Partnership Marketplaces - Name of Organization: R6 Regional Council Consortium Name: Aging/Volunteer Services Organization Type: Local Health and Human Services Annual Funding Amounts: August 27, 2024 to August 26, 2025- $75,000 August 27,2025 to August 26, 2026 -$75,000 August 27,2026 to August 26, 2027 $75,000 August 27, 2027 to August 26, 2028 $75,000 August 27, 2028 to August 26, 2029 $75,000 Name of FFE state: Utah in Six Rural Counties (Juab, Millard, Piute, Sanpete, Sevier, and Wayne Counties) Priority Populations: Rural Communities, Low-income or unhoused individuals, People with physical or intellectual disabilities, and Medicaid-eligible consumers who are not enrolled in coverage despite being eligible for Medicaid. Total Number of Navigator staff: 2 staff members. We will utilize volunteers to assist with outreach and education and plan to utilize 5 volunteers. but 2 staff members will be trained by October 1, 2024. The purpose of this grant is to reach out to underserved and/or vulnerable populations and provide the uninsured with affordable health coverage. R6 Regional Council (R6) will target populations through various marketing efforts to provide needed services by dispensing information, education, outreach, and services in a non-biased, fair, accurate, and impartial manner. R6 will assist consumers with complex cases, coverage information, tax information related to marketplace, exemptions, and/or appeals. R6 staff and volunteers will form partnerships with various agencies who target the Priority Populations to distribute information about the Exchange. Objectives of this grant are to establish relationships with local healthcare and community agencies to offer and promote Navigator services. Staff and Volunteers will conduct targeted outreach, education, and enrollment activities. Due to these objectives, the expected outcomes are to reach 4,000 community members through outreach and enrollment events (hybrid, in-person, and/or virtually). Have a 60% increase in their knowledge about Marketplace Insurance and/or resources available, Reach 100 people with enrollment in marketplace insurance and/or receive resources to meet their needs. Build new relationships with 15 agencies within the Region and Co-host 30 events.
