Choose Healthy Life will activate a faith-based network in South Carolina to conduct outreach, education and health navigation services to enroll Black Americans in affordable health insurance plans. - Choose Healthy Life, a community-based and faith-based nonprofit 501(c)3 organization, proposes the Black Clergy Action Plan to provide targeted and culturally competent health navigation services for at risk and socially-vulnerable Americans in South Carolina with a specific focus on uninsured Black and African Americans. Our partners will include The Charity Foundation, Charity Missionary Baptist Church, St. John Baptist Church, Israel CME, Tabernacle Baptist, Victory Tabernacle Deliverance Temple, New Grace United Methodist, Freedom Temple Ministries, Brookland Baptist Church, and Mt Carmel United Methodist Church, as well as community-based groups such as Order of Eastern Star, Electa Chapter, The Links, The Elks, Jack and Jill of America, International Ministerial Alliance (IMA), International African American Museum (IAM), and Charleston Area Justice Ministry (CAJM). This network will drive community awareness and outreach and the named Black churches will provide 1:1 navigation services with a total of nine (9) trained and certified health navigators who are representative of the priority population and one (1) health navigator supervisor who will also be trained and certified. To reach the priority population of uninsured rural, hard to reach Black and African Americans in South Carolina, the proposed Federally-Facilitated Exchange (FFE) service areas are fifteen (15) counties in South Carolina: Allendale, Bamberg, Barnwell, Beaufort, Charleston, Colleton, Darlington, Dorchester, Greenville, Hampton, Lexington, Orangeburg, Richland, Spartanburg, and York. These counties are those with the highest number of uninsured Black Americans and with the highest health disparities. The annual funding amounts for the twelve-month budget periods for Years 1-5 are: Year 1 $1,159,796.00 Year 2 $1,192,810.00 Year 3 $1,221,304.00 Year 4 $1,249,747.00 Year 5 $1,279,041.00 TOTAL REQUEST $6,102,698.00
