NH Health Insurance Navigators - Organization Name: Foundation for Healthy Communities Organization type: Community and consumer-focused nonprofit. Annual funding amounts for each 12-month budget period Years 1-5: $1.5M Name of the FFE state(s): New Hampshire Proposed priority populations to be served: Low-income individuals, racial and ethnic minorities, rural communities, LGBTQ+ individuals, people with a mental health or substance use disorder, pregnant women, new mothers, and women with children. Other priority populations will be identified as appropriate throughout the 5-years. Total number of Navigator staff your organization will have federally trained and certified or re-certified for PY 2025 by October 1, 2024: 5 Project Purpose: To expand upon the services offered by the Foundation for Healthy Communities to health insurance consumers throughout the state of New Hampshire both in person and virtually via a telephone/video helpline. Provide a dedicated focus on priority populations and assist them with enrollment, education, and addressing social determinants of health. Summary of Proposed Project: The Foundation for Healthy Communities will hire a Senior Director, Director, and three Coordinators, who will all be Certified Marketplace Navigators. FHC will (1) Provide comprehensive in-person and phone/virtual Navigation services to all residents of New Hampshire; (2) Conduct statewide outreach to all residents with a focus on priority populations and community relationship building; (3) Maintain strong partnerships with organizations serving individuals impacted by substance use and mental health disorders, primary care and hospital systems, women and children health, poverty and rurality, and minority populations; (4) Build partnerships with statewide entities serving those in need of Navigation services & (6) Refer consumers to needed social services to reduce health barriers. The Foundation for Healthy Communities along with all Navigators will adhere to the ACA conflict of interest policies. FHC will contract with First Choice Services to maintain and expand current telephonic enrollment support to all consumers in New Hampshire. Outcomes: Throughout the 5-year cooperative agreement, the Foundation for Healthy Communities will (1) Employ 5 Federally Trained and Certified Navigators and contract with First Choice Services to employ 6 full time and 3 part time Navigators; (2) Provide 8500 1:1 interactions between Navigators and consumers; (3) Assist 3000 consumers with enrollment or re-enrollment in a Qualified Health Plan; (4) Assist 2400 consumers with Medicaid/CHIP applications or referrals; (5) Assist 2000 consumers with understanding the basic concepts and rights related to health coverage and how to use health coverage; (6) Assist 400 consumers with complex cases, other Exchange (Marketplace) enrollment issues, or referrals; (7) Assist 75 consumers with Marketplace forms, exemptions, and appeals & (8) Analyze outcome and service data by priority populations to ensure equal access, service provision & outcomes.
