Cooperative Agreement to support Navigators in Federally facilitated exchanges. Genesee Health Plan will use its "Get It, Keep It, Know It, Use it" campaign to educate/enroll priority populations. - Genesee Health Plan (GHP) is a 501c3 Nonprofit organization in Flint, Michigan, with service areas in Genesee, Saginaw, and Bay Counties. For over 20 years, GHP has provided health care coverage enrollment and community resources to individuals in these service areas. The annual funding amounts for each 12-month budget period (years 1-5) total $368,472. 22 GHP Navigator Staff will be federally trained by October 1, 2024. GHP has partnered with CMS for over 5 years as a CMS Navigator Grantee. Whether receiving a written recognition by the Secretary of Health and Human Services for our work on the water crisis, participating with CMS staff on media interviews and press conferences, or sharing examples of our media with CMS staff to share with other assisters in the country, we have valued our partnership with CMS. For this project, GHP plans to target the following priority populations in all service areas: • Medicaid-eligible consumers who are not enrolled in coverage despite being eligible for Medicaid. Medicaid Unwinding caused many beneficiaries to lose their coverage due to technical error, confusion, health insurance literacy, not receiving their renewal packet, and not being redetermined; some of whom are still eligible but not enrolled in Medicaid. • Low-income or unhoused individuals. Families have become more displaced and lack adequate affordable housing, they are having to choose between paying for groceries, utilities or prescriptions and other essential basic needs. • Racial and ethnic minorities. GHP is positioned within the faith-based community, local nonprofits, and several food pantries, to assist racial and ethnic minorities, including Black/African American, Hispanic/Latino, and Arab Americans. • Individuals with cognitive, hearing, speech, and/or vision impairments. Populations include individuals with cognitive, hearing, speech, and vision impairments. • Rural communities. GHP employs a team of community health workers (CHWs), who are also certified navigators, assisting community members who live in rural areas. Through our work as a CMS grantee, GHP staff gained knowledge about the Social Determinants of Health (SDOH) within our priority populations in the service areas where they reside. GHP has identified and evaluated the following SDOH as they relate to the priority populations we plan to serve: • Access to clean water • Basic needs • Transportation • Language and culture Additionally, GHP will balance efforts between the three different counties using a mixture of social media, current Medicaid beneficiary numbers in each county, utilizing the MAC portal, and addressing the most priority consumers with extenuating circumstances in each area. Through partnerships with other community organizations, GHP CHWs and navigation staff will continue to conduct 1:1 enrollment appointments with priority populations at each service area.
