ACCESS Health Insurance Navigators for Michigan - The Arab Community Center for Economic and Social Services (ACCESS) of Dearborn, Michigan submits this application to the Centers for Medicare and Medicaid Services for a Cooperative Agreement to Support Navigators in Federally-facilitated Exchanges (CA-NAV-24-001). The program, “ACCESS Health Insurance Navigators for Michigan” is coordinated by ACCESS. The collaborative approach brings together subrecipient navigator organizations across the state of Michigan, to reach the entire state, including multiple priority populations. The proposed annual funding amounts per project year are: $2,850,176 (2025); $2,850,176 (2026), $2,850,176 (2027), $2,850,176 (2028), $2,850,176 (2029). Priority populations include racial and ethnic minorities, LGBTQ+ individuals, people with disabilities, people with impairments, pregnant women, new mothers, women with children, individuals with SMI/SUD, individuals with HIV, individuals with behavioral health conditions, and limited English proficient individuals in the state of Michigan. Subrecipient organizations for this program are Association of Chinese Americans, Community Health Access Coalition – Chippewa County, Community Welfare Services of Metro Detroit, Detroit Parent Network, Harbor Beach Community Hospital, Jewish Family Services, Michigan 2-1-1, Refugee Development Center, Ruth Ellis Center, and the Washtenaw Health Plan. The program also includes Michigan Primary Care Association as a subrecipient, which will coordinate navigator services among 11 federally qualified health center second-tier subrecipients across Michigan. The total number of Navigator staff ACCESS and its subrecipient partners will have federally trained and certified / recertified for PY 2025 by 1 October 2024 is 38. ACCESS is a 501(c)3 organization founded in 1971 and registered in the State of Michigan. ACCESS is an ethnic community-based organizations that addresses the economic and social self-sufficiency and health and wellbeing of underserved individuals living in Southeast Michigan/greater Detroit.
