Get Covered Collaborative will provide Navigator services statewide in South Dakota. - Cooperative Agreement to Support Navigators in Federally-Facilitated Exchanges Funding Opportunity Number: CMS-NAV-24-001 Applicant Organization: Community HealthCare Association of the Dakotas Organization Type: Community and consumer-focused 501(c)(3) nonprofit organization Consortium: Get Covered Collaborative FFE State: South Dakota Community HealthCare Association of the Dakotas (CHAD) will partner with subrecipient organizations to continue the work of the Get Covered Collaborative (GCC) to provide statewide health insurance education and outreach, enrollment assistance, and post-enrollment services to consumers in South Dakota. Priority populations include rural communities, low-income individuals, American Indians, racial and ethnic minorities, lesbian, gay, bisexual, transgender, queer and other LGBTQ+ individuals and Medicaid-eligible consumers who are not enrolled in coverage. CHAD serves as the Primary Care Association in South Dakota and supports the work and mission of community health centers that provide quality health care to all patients, especially those in rural locations, with low-income, and those with identities that are traditionally marginalized. CHAD is a 501 (c)(3) nonprofit entity that will serve as the grantee, lead consortium applicant, fiscal agent, and manager for the Get Covered Collaborative. The Get Covered Collaborative will train and certify 17 Navigators by October 1, 2024.
