COOPERATIVE AGREEMENT TO SUPPORT NAVIGATORS IN FEDERALLY-FACILITATED EXCHANGES - ASPIN HEALTH NAVIGATORS - Project Title: ASPIN Health Navigator Program Program: CA-NAV-24-001 Applicant: Affiliated Service Providers of Indiana, Inc. d/b/a ASPIN (Community/consumer focused nonprofit) Congressional Districts: IN - All Project Summary: The ASPIN Health Navigator Program will establish new growth, build on previous years using data collected from individuals calling the call center and from consumers’ input at outreach events. ASPIN proposes to provide innovative enrollment, reenrollment and education over the next five years leading to greater access to healthcare options. Under this contract application, ASPIN will implement new initiatives and build on old methods that worked to reach priority populations and anyone in the state needing help with enrollment. ASPIN Navigators partner with over 50 agencies: WIC, Exodus Refugee, Women’s Care Center, Purdue Ext., county health departments. ASPIN is partnered with IU Health to host enrollment events at churches through the state: Burmese and Spanish events each had over 100 attendees. ASPIN has attended Pride events including Indy Pride, Spencer Pride, St. Joseph Pride. Priority Populations in Indiana ASPIN will target while sustaining previous partnerships: New Mothers, Children and Families, Minorities, LGBTQ+, Veterans, Immigrants, Rural, Criminal Justice involved, Homeless individuals and Medicaid Eligible consumers: enrolling both in-person and virtually. Project Goals: 1.Number of Navigators to be certified/re-certified by October 1, 2024 will be 15 FTE & 2 PTE (Data Analyst @ 0.25 FTE & Office Asst. @ 0.75 FTE). 2.Number of consumers expected to be reached through education and outreach activities. Assist 250,000 during the first-year contract period. 3. Number of 1:1 consumer interaction during 2024-2025 budget period through toll-free phone line (1-877-313-7215) manned by trained navigators 8:30 am to 4:30 pm five days per week with translation capabilities in 100+ languages. Projected through Call Center and follow-up appointments, 40,000 1:1 consumer interactions during the first year contract period. 4. Enrollment and re-enrollment in a QHP: Using mobile offices or meeting virtually, ASPIN navigators will have the flexibility to establish web interface for plan exploration, selection, document sharing, and application submission. Number of consumers projected to assist through enrollment 1,200 for the first year of the contract period. 5. Number of Consumers assisted with Enrollment or Reenrollment in Medicaid/CHIP Provide information and services in fair, accurate, impartial manner to help consumers make informed decisions about Medicaid, CHIP and HIP Number of consumers projected to assist through enrollment 5,760 for the contract period. 6. Number of consumers assisted with understanding the basic concepts and rights related to health coverage and how to use it: Through educational workshops, ASPIN Navigators will assist 2,500 consumers in understanding benefits of healthcare. 7. Number of consumers assisted with complex and other marketplace enrollment issues: In 1st contract year, Navigators will assist with 500 complex case/other marketplace enrollment issues. 8. Number of consumers assisted with marketplace forms, exemptions, and appeals: Assist 25 consumers in correcting errors or appealing wrongful denials.
