Cover Kansas Navigator Project 2024-2029 - Community Care Network of Kansas (Community Care) is the applicant for the Cover Kansas Navigator Project and is a current recipient of funding for the Cooperative Agreement to Support Navigators in Federally-Facilitated Exchanges 2021-2024. The Project will use a statewide network of federally qualified health centers and other community-based clinics, as well as two Virtual Navigators, to expand the reach of the Project to priority populations and other Kansans seeking assistance with the Federally facilitated Exchange (FFE). Community Care, the HRSA designated primary care association in Kansas, is a community and consumer-focused nonprofit and will not utilize any sub-recipients or sub-awardees. Annual funding amount requested for each 12-month period (Years 1 through 5) is $1,500,000, totaling $7,500,000 for the 60-month period of performance from August 2024 to August 2029. The Cover Kansas Project will be statewide, serving all 105 Kansas counties, with a focus on the 386,291 individuals within priority populations listed below. Community Care health centers serve patients without regard to ability to pay and specialize in providing care to uninsured, underinsured, and other vulnerable populations. The Cover Kansas Project will target underserved or vulnerable populations, especially those less likely to be familiar with the FFE. Additionally, priority populations of focus include 1) uninsured individuals residing in 34 rural and frontier counties in western Kansas, including 24,726 adults; 2) 142,065 Kansans may who qualify for Marketplace subsidies; 3) approximately 90,500 individuals who are eligible for Medicaid and lost or are at risk of losing coverage; and 4) 129,000 members of the LGBTQ+ population across the state. Navigators strategically placed at Community Care clinic locations throughout the state are well-positioned to provide culturally and linguistically appropriate assistance to vulnerable populations living in underserved communities. Navigators will utilize the tools such as the Marketplace Assister Community (MAC) to provide outreach to consumers who have lost Medicaid coverage to connect them with Marketplace information and application assistance. The following list describes the breakdown of federally trained and certified or recertified Navigators for Program Year 2025 by October 1, 2024. a. At minimum, 30 Navigators will be trained and certified or re-certified by Oct. 1, 2024. b. 5 Navigators will be paid full-time/100% from Navigator grant funds. c. 0 Navigator(s) will be paid part-time from Navigator grant funds. d. At minimum, 25 Navigators will be volunteers/not paid from Navigator grant funds. Navigators in line (d) above will work in health centers and clinics within the Community Care Network, assuring a physical presence in the communities they serve, and will travel to surrounding counties as capacity allows. Cover Kansas will utilize two Virtual Navigators who will assist consumers remotely via phone or secure video call on the Eastern and Western regions of the state, respectively.
