Community - Access to Care - Navigator 2024-2029 - MHP Salud, a community and consumer-focused non-profit, requests to continue operating its Navigator program, which is grounded in the Community Health Worker (CHW) model, through this new funding opportunity. For the new project cycle, the program will target Hispanic/Latino communities, populations with limited English proficiency, uninsured individuals, and rural communities in 67 counties across South Texas (Aransas, Atascosa, Bandera, Bee, Bexar, Blanco, Brewster, Brooks, Caldwell, Calhoun, Cameron, Colorado, Comal, Crockett, Culberson, DeWitt, Dimmit, Duval, Edwards, El Paso, Frio, Gillespie, Goliad, Gonzalez, Guadalupe, Hays, Hidalgo, Hudspeth, Jackson, Jeff Davis, Jim Hogg, Jim Wells, Karnes, Kendall, Kenedy, Kerr, Kinney, Kleberg, La Salle, Lavaca, Live Oak, Loving, Matagorda, Maverick, McMullen, Medina, Nueces, Pecos, Presidio, Real, Reeves, Refugio, San Patricio, Starr, Sutton, Terrell, Uvalde, Val Verde, Victoria, War, Webb, Wharton, Willacy, Wilson, Winkler, Zapata, and Zavala counties). With a combined population of 6,145,588, 67.4% of whom identify as Hispanic/Latino, this region faces significant challenges: 17.9% are uninsured, 18.4% live in poverty, and 42% speak a language other than English at home (U.S. Census Bureau). In addition to serving the priority populations noted above, MHP Salud has strategies in place to promote the Marketplace and Navigator services to the broader uninsured and currently insured population in the proposed FFE, including communities that may have been underserved during previous open enrollment periods and individuals affected by Medicaid unwinding. The project’s core objective is to meet goals established in the 2025-2029 Navigator Project Goals, including assisting 3,270 consumers with enrollment or re-enrollment in a QHP and assisting 2,525 consumers with Medicaid/CHIP applications or referrals. During the first year of the project, there will be 28 full-time Navigators in Texas, 25 of whom are existing staff who will be federally trained and re-certified for PY 2025 prior to October 1, 2024. The Navigators will be supervised by 6 Program Managers and 2 Program Directors. All new Navigators hired throughout the project year will be federally trained and certified prior to providing any Navigator services. In addition, all Navigators on staff are bilingual (English and Spanish). Navigators will continue to provide services post-open enrollment. MHP Salud’s Navigator program will not use volunteers, part-time staff, or subrecipients during this budget period. All services and materials MHP Salud offers are bilingual (English and Spanish) and staff are prepared to provide support that is culturally and linguistically responsive to consumers’ needs. To ensure MHP Salud uses federal funds efficiently and to reach the maximum number of consumers possible throughout the FFE, MHP Salud will combine in-person and virtual enrollment assistance. Virtual assistance will be provided over the phone and through videoconferencing. Additionally, marketing efforts will be tailored to priority populations in each region to ensure consumers are aware of MHP Salud’s Navigator services and to connect individuals with Navigators for assistance. For in-person assistance, MHP Salud provides Navigator services that are accessible and conveniently located for consumers. MHP Salud will continue to ensure its marketing and consumer education materials are accessible and inclusive, in part by using a variety of mediums (e.g., auditory, visual) to advertise the Marketplace and the availability of Navigator services across the FFE.
