Cooperative Agreement to Support Navigators in Federally-facilitated Exchanges - Cooperative Agreement Activities: The cooperative agreement will perform Navigator functions as outlined in 45 CFR � 155.210 and � 155.215. The ENTX Consortium, made up of four organizations in Dallas-Fort Worth Metropolitan area (United Way of Metropolitan Dallas, CitySquare, LBU Community Clinic and Prism Health North Texas), will incorporate various approaches and activities to promote enrollment through community leadership and extensive collaborations. The organizations will implement outreach, maintain a unified and diverse network of Navigators and plan strategically for open enrollment periods to maximize access and efficiency in the enrollment process. Navigators will help targeted populations to compare health plans, review benefits, select QHPs, and apply for subsidies. Navigators will also assist consumers with re-enrollment. The original consortium also included Community Council of Greater Dallas (CCGD), who has since decided to leave the consortium. United Way will reallocate those sub-grantee funds within the active consortium members and transition existing certified Navigators to other consortium sub-grantees, increase marketing to continue to reach those target populations, and reach the proposed consortium goals. Budget Year Two: For 12 months, $2,661,467 will fund a minimum of 39 Navigators for the following ENTX Consortium members: Lead agency, United Way of Metropolitan Dallas ($616,999); CitySquare ($713,068), LBU Community Clinic ($451,400), and Prism Health North Texas ($880,000). Total Amount Requested: $2,661,467 for the 2022-2023 year. Target Population: The ENTX Consortium selected target populations based on the specific needs in the geographic region and the expertise of each consortium member. The ENTX Consortium intends to target the following vulnerable populations: " Racial/ethnic minorities and individuals with limited English proficiency " Lesbian, gay, bisexual, transgender, questioning, intersex, asexual, and other (LGBTQIA+) individuals " Low income, economically disadvantaged, and homeless " Pregnant women, children, and women with children " Individuals with mental health or substance-related disorders " Individuals with HIV/AIDS " Youth transitioning from substitute care Counties Served: Collin, Cooke, Dallas, Denton, Ellis, Fannin, Grayson, Henderson, Hunt, Kaufman, Navarro, Rockwall, and Tarrant counties, all located in North Texas. Goals: " Reach at least 2,900,000 consumers through education, enrollment and outreach activities. " Provide 16,200 consumers with 1:1 assistance in response to inquiries. " Assist 2,000 consumers with enrolling or re-enrolling in a QHP. " Assist 7,500 consumers with Medicaid/CHIP applications or referrals. " Assist 23,000 consumers with understanding the basic concepts and rights related to health coverage and how to use it. " Assist 6,500 consumers with complex cases and other marketplace enrollment issues. " Assist 2,500 consumers with Marketplace forms, exemptions, and appeals. " Train and certify or re-certify at least 34.3 FTE (39 total) Navigators by October 1, 2022.
