The goals of Ascension Navigator Project-budget year 3 (2023-2024) are: 1) provide 325 public events raising awareness about FFEs and helping consumers understand their health coverage options; 2) reach 21,000 consumers through marketing and promotional activities; 3)provide 1:1 assistance to 5500 consumers; 4)enroll 275 consumers in a qualified health plan (QHP); 5) assist 450 consumers with Medicaid/CHIP applications or referrals; and 6) have 5 FTE and 3 part-time certified/recertified Navigators by 10/01/2023 and throughout the 2023-2024 budget period.
Ascension will perform all Navigator duties. No subrecipients or contractors will be used for the project.
Ascension requests the following yearly budget amounts: Indiana $605,053
Ascension Indiana Navigator Program will serve every consumer in need of assistance. However, a special emphasis will be placed on serving and aiding the historically underserved and vulnerable. These historically underserved and vulnerable populations include: 1) Latinx, African Americans, and other historically underserved ethnic and racial minorities, 2) residents from rural communities, 3) impoverished residents, 4) residents lacking reliable transportation and access to high-speed internet, 5? unemployed residents, 6) how wage, "gigeconomy" and self-employed residents, 7) residents with chronic illness including mental illness and HIV/AIDS, 8) residents battling cancer, 9) families of newborns. The size of the underserved and vulnerable populations is approximately: Indiana - 940,000; During the project's third year, Ascension will reach 21,000 underserved residents in Indiana.
Ascension will target the following Indiana counties- Clay, Clinton, Fountain, Hamilton, Hendricks, Howard, Jennings, Lawrence, Madison, Marion, Randolph, Vanderburgh, Warren, Warrick and Washington.
Ascension will have by 10/01/2023, 5 active fill-time certified/recertified Navigators and 3 part-time certified/recertified navigators throughout open enrollment.
Ascension will have 5 active full-time certified/recertified Navigators post enrollment and 3 part-time certified/recertified Navigators post enrollment.
