Cooperative Agreement to Support Navigators in Federally-facilitated Exchanges - Project Abstract Summary Version 2.0

Southwest Utah Community Health Center, dba Family Healthcare (FHC) is the lead awardee and will be providing subrecipient agreements to the six other Consortium health centers. The Consortium includes Bear Lake Community Health Center, Inc.; Carbon Medical Services Association, Inc.; Mountainlands Community Health Center (dba Mountainlands Family Health Center); Utah Partners for Health; Wasatch Homeless Health Care, Inc. (dba Fourth Street Clinic); and Wayne Community Health Centers, Inc.
Total funding amount for the third budget period.
The proposed total funding allocation available for the third and final 12-month budget period (August 27, 2023 to August 26, 2024) under the 2021 Navigator NOFO is $1,290,207.00. The proposed total funding includes a one-time proposed allocation of $250,000.00 that will be used to support additional outreach, education, and enrollment activities necessary to facilitate coverage transitions for consumers impacted by Medicaid unwinding during the 2023-2024 budget period.
Population(s) or group(s) that will be targeted.
Consortium health centers are open to all regardless of ability to pay, however, services target the low-income population living at or below 200% Federal Poverty Guidelines (FPG), underserved communities, the population experiencing homelessness, communities of color, tribal populations, individuals with limited English proficiency, and individuals with disabilities residing in Federally Facilitated Exchange (FFE) Counties in Utah.

Beaver, Box Elder, Cache, Carbon, Daggett, Davis, Duchesne, Emery, Garfield, Iron, Juab, Kane, Millard, Piute, Rich, Salt Lake, San Juan, Sanpete, Sevier, Tooele, Uintah, Utah, Wasatch, Washington, Wayne, and Weber serving 26 out of 29 Utah FFE counties. not include bear yet
