Goals LifeSpring Health Systems will enroll 1,000 consumers in its 11-county rural service area in Medicaid/CHIP or QHPs during year 2 of this grant period. This breaks down to 300 individuals in Marketplace QHP and 700 Medicaid or CHIP. This combined enrollment constitutes 1:1 engagement of 1,000 individuals. Through marketing and promotional activities including print ads, social media, community events, employer presentations, health fairs and other outreach activities, LifeSpring will provide information regarding Healthcare Marketplace plan enrollment to at least 6,000 individuals. Population To Be Served: Vulnerable populations including: uninsured, homeless, Spanish-speaking, Medicaid eligible, LGBTQ+, individuals with mental illness and substance use disorders, minorities, rural, pregnant women, low income, individuals with HIV/AIDS.LifeSpring currently has 9 FTE, State and Federally trained and certified Navigators plus the addition of 6 Navigators, both State and Federally trained and certified.
