The Missouri Coalition for Primary Health Care d/b/a Missouri Primary Care Association (MPCA) is a non-profit organization dedicated to improving access to high quality, community-based, and affordable primary health care in Missouri. MPCA and partnering FQHCs request $2,903,500 in year 3 Navigator funding to serve Missouri and continue the Show Me Coverage program - a statewide Navigator program serving residents of Missouri with a target of reaching vulnerable populations, assisting both FQHC patients as well as members of the community. Goals of the project include: 1) Continue Show Me Coverage statewide Navigator program with 20 regional FQHC subrecipients; 2) Provide outreach, enrollment, and education to 78,720 consumers across the state to raise awareness about FFEs and help consumers understand coverage options; 3) Provide marketing and promotion activities through social media impressions, viewership, listenership, etc. to 700,000 consumers; 4) Provide 24,535 consumer 1:1 assistance from Show Me Coverage Navigators; 5) Enrolling or re-enrolling 1,250 consumers in a QHP and an estimated 11,000 in Medicaid/CHIP applications and referrals; 6) Ensure access to Navigator services in Missouri through subrecipient FQHCs that by October 1, 2023 employ a) 37.79 FTE Navigators, b) 29.0 FTE full-time Navigators, c) 27 part-time Navigators for 8.79 FTE, d) 0 FTE volunteers. Throughout the 2023-2024 budget period Show Me Coverage includes the following federally-trained and certified/re-certified Navigators available to assist consumers: a) 37.79 FTE Navigators, b) 29.0 FTE full-time Navigators, c) 27 part-time Navigators for 8.79 FTE, d) 0 FTE volunteers. Subrecipients that will perform Navigator functions include 20 Missouri Federally Qualified Health Centers: Access Family Care; Affinia Healthcare; Arthur Center; CareSTL Health; Central Ozarks Medical Center; Clarity Healthcare; Compass Health Network; Comtrea; Family Care Health Centers; Fordlan
d Clinic; Four Rivers Community Health Center; Great Mines Health Center; Health Care Collaborative; Katy Trail Health Center; KC Care Health Center; Missouri Highlands Health Care; Ozarks Community Health Center; Betty Jean Kerr Peoples' Health Center; Samuel Rodgers Health Center; and Southeast Missouri Health Network. MPCA and its subrecipients will target uninsured and vulnerable Missourians through a statewide approach. Targeted vulnerable populations include racial and ethnic minorities; rural communities; LGBTQ+ individuals; low income or homeless individuals; American Indians and Alaska Natives; people with physical or intellectual disabilities or cognitive, hearing, speech, and/or vision impairments; pregnant women, new mothers, and women with children; individuals with mental health or substance-related disorders; individuals with HIV/AIDS; and Medicaid-eligible consumers who are not enrolled in coverage despite being eligible. The cooperative agreement will be used to perform Navigator functions, as outlined in 45 C.F.R. Sec. 155.210 and Sec. 155.215. A coordinated communications plan will continue, providing targeted outreach messaging. To reach uninsured and vulnerable populations, MPCA and its subrecipients will collaborate with school districts, homeless shelters, health departments, hospitals, Ryan White HIV/AIDS Program grantees, Job Corps and Workforce Development Centers, colleges and universities, Chambers of Commerce, food banks, local tax services, libraries, YMCAs, Boys and Girls Clubs, Area Resource Councils, faith-based organizations, minority support organizations, LGBTQ+ support organizations, the Family Support Division of the Missouri Department of Social Services, local area mental health centers, Community Action Agencies, Disability Supports, and the Missouri Department of Corrections.
