Foundation Communities is a 501c3 nonprofit organization based in Austin, TX that provides affordable housing and support services for underserved communities. We seek to expand Affordable Care Act (ACA) education and enrollment for underserved and historically vulnerable populations in Central Texas.
Our priority populations for Navigator activities are racial and ethnic minorities; non-English speaking populations; documented immigrants to the United States; rural communities; LGBTQ+ individuals; low-income individuals and households and those who fall into Texas' Medicaid gap; low-income or low educational attainment individuals undergoing cancer treatment; individuals experiencing homelessness; individuals leaving incarceration; young people over age 18 without family coverage; young people turning 26 years old; international students with F or J visas; pregnant women, new mothers, and women with children; individuals with mental health and/or substance-related disorders; and individuals living with HIV/AIDS.
Our enrollment, education, and outreach efforts will target those underserved and vulnerable populations in the following 12 FFE counties (listed with percentages of uninsured): Bastrop 28%, Bell 15%, Blanco 22%, Burnet 13%, Caldwell 28%, Fayette 28%, Gillespie 22%, Hays 14%, Llano 13%, McClennan15%, Travis 14%, Williamson 10%.
Continued Navigator funding will allow Foundation Communities Prosper Health Coverage program (PHC) to expand our outreach, education, promotion, enrollment, utilization, and other assister services by funding our team of Navigators and support staff.
In Year 3 will we boost current outreach, enrollment, and support efforts to target populations through culturally tailored media campaigns, partnership development with community organizations that advocate for and serve members of these underserved and vulnerable populations within our proposed FFE service area, and multilingual advertising in high-volume public services.
We will provide information and services in fair, accurate, and impartial manners with population-specific cultural and linguistic appropriateness; educate populations on enrollment options; facilitate clients in selecting qualified health plans; inform clients about their options for qualified or commensurate health coverage options; increase health enrollment and health coverage literacy in Texas; help clients use their health plans to their best benefit; and increase health equity and health outcomes among underserved and vulnerable populations.
