The Arizona Association of Community Health Centers (AACHC, dba Arizona Alliance for Community Health Centers), located in Phoenix, Arizona, seeks to expand on its current successful statewide Navigator program, "Connecting Arizonans to Coverage (CA2C)," for budget year three (2023-2024) with the anticipated funding amount of $3,299,160.
Goals for the 2023-2024 budget period include 74 fully trained Navigators by October 1, 2023, ten Navigators will be trained Post Open Enrolment through the end of the budget year, August 26, 2024. Six million consumers will be reached through marketing and promotional activities. Navigators will assist 2,300 or more consumers enroll in a Marketplace health plan and an additional 21,000 enroll in Medicaid. Navigators will also assist 10,700 consumers understand the basic concepts and rights related to heatlhcare coverage and how to use it. One hundred and ten consumers will be assisted with complex cases, and another 140 consumers will receive assistance with Marketplace forms, exemptions, and appeals.
CA2C members include local community-based outreach and enrollment groups, Community Health Centers (CHCs), and the Arizona Center for Rural Health. Entities receiving grant funds as subrecipients and providing Navigator services include Adelante Healthcare; Arizona Center for Rural Health (University of Arizona); Asian Pacific Community in Action; Chicanos Por La Causa- Keogh Health Connection; Chiricahua Community Health Centers, Inc.; El Rio Health; Helping Families in Need; Neighborhood Outreach Access to Health; North Country HealthCare; Sun Life Health; and Sunset Health.
The population(s) or group(s) that will be targeted for 2023-2024 will continue to be individuals living in all 15 counties in Arizona. Special focus will be placed on connecting those who may be uninsured, underserved, and vulnerable to insurance for Medicare/Medicaid or Marketplace insurance. AACHC will also implement outreach and enrollment efforts towards the end of Medicaid continuous coverage to help mitigate loss of health coverage. CA2c members serve all who seek assistance regardless of level of English proficiency or disability and serve a wide variety of underserved people who may be hard to reach.
