Consortium Members: Consortium members include Community HealthCare Association of the Dakotas (PCA); four federally qualified community health centers (FQHCs); Community Health Center of the Black Hills, City of Sioux Falls dba Falls Community Health, Horizon Health Care, South Dakota Urban Indian Health; one community action agency; and the 2-1-1 community referral resources in SD. All consortium members are 501 (c)(3) nonprofit entities governed by boards of directors. All consortium members will be paid with Navigator cooperative agreement funds to hire and house Navigators who will perform Navigator activities to serve consumers across South Dakota, with a focus on serving rural and underserved populations.
Funding Requested: Community HealthCare Association of the Dakotas is applying for $1,100,000 to continue administering the project for the 2022-2023 12-month anticipated period of performance. Cooperative agreement funds will be used to hire/maintain active Navigator staff to carry out the project goals, assisting consumers through education, enrollment, and outreach activities during the entire 2023-2023 budget period.
Populations of Focus and Service Area: The Get Covered Collaborative will continue to provide comprehensive and accessible Navigator services to consumers statewide in SD. Underserved or vulnerable populations of focus will include rural communities, low income or persons unstably housed, racial and ethnic minorities, American Indians, LGBTQ+ community, individuals with mental health or substance-related disorders and the uninsured/underinsured. Health center partners serve more than 90,000 patients per year in 33 communities and 43 delivery sites. 21% of health center patients in South Dakota are uninsured, while 81% have incomes less than 200% federal poverty level.
Number of active Navigators: Currently, 11 active Navigator staff (63% full-time / 27% part-time) are federally trained and certified. The same number of active Navigator staff will be re-certified by October 1, 2022 and throughout open enrollment.
