Swope Health Services (Swope Health) is the single recipient of CMS NAVCA210393 grant funds and all cooperative agreement activities are conducted solely by Swope Health employees. Our Year 2 funding request to deliver FFE services to residents of four Missouri counties is $249,221.00.COUNTIES SERVED: Cass, Clay, Jackson and Platte counties. PROJECT DESCRIPTION: Our Year 3 activities include pre-enrollment marketing across the four MO counties to increase consumer awareness of the pending Open Enrollment period, SEPs and the Unwinding and to encourage them to access enrollment assistance. We will use multiple methods such as radio advertising, social media and website postings, printed materials and presentations and participation in collaborative events to sustain consumer awareness. Navigators will provide direct enrollment assistance at some community outreach events. Navigators will be consistently available to provide education, enrollment and post-enrollment assistance to consumers in-person at two accessible Swope Health clinics and by telephone. We will work with our existing and new collaborators across the four counties to implement CMS Themed Week activities and messaging for consumers. Our goal is to reach 8,000 consumers through education, outreach and enrollment activities during Year 3, and directly interact with 3,500 of them during the project year. We plan to assist 55 consumers with the selection of a MO FFE QHP during Year 3 and assist 250 consumers with Medicaid / CHIP applications. The non-grant funded Project Director will lead the forging of new collaborations, assure conflict of interest and security requirements are met and continuously update in-house Navigator fact sheets to ensure that Navigators are familiar with all QHP and MO Medicaid/CHIP changes and support the Navigators in addressing complex cases. NUMBER OF NAVIGATORS: Swope Health employs three (3) fulltime, CMS grant-funded Navigators. The three fulltime em
ployees are trained and certified by CMS and the State of Missouri. The three Navigators will begin the recertification process with CMS and the State upon release of the re-certification training with the objective to complete all certifications by October 1, 2023, well before the start of Open enrollment on November 1,2023. NUMBER OF ACTIVE NAVIGATOR STAFF: We do not anticipate the termination of any of the three Navigators before January 15, 2024, the end of the Open Enrollment. Should this unforeseen event occur Swope Health will hire, onboard and assure the CMS and State certification of any newly hired Navigator. The three Navigators will provide post-enrollment assistance to consumers throughout the grant period ending August 26, 2024.
