The Health Center became a Federally Qualified Health Center (FQHC) Look-alike in 2007 and achieved full FQHC status in 2015, providing the area with needed medical, dental, mental health, and enabling services. Fordland Clinic’s mission is to improve the health of their community, and they have a vision to serve as a model for quality, affordable healthcare by offering comprehensive medical, dental, mental health, prevention, and wellness services to everyone regardless of insurance and income. Fordland Clinic served 7,221 unduplicated patients in 2023, providing 802 patients with mental health services and 0 patients received substance use disorder services. BHSE funding will allow for the increase behavioral health services including both mental health and substance use disorder services. Fordland Clinic offers comprehensive behavioral health services designed to support individuals and their families at every stage of their life. Fordland Clinic understands that different age groups have unique needs ensuring that services are tailored to cater to specific developmental stages. Behavioral health services are offered at the Fordland and Kimberling City clinic locations, as noted in Form 5B. Specialized behavioral health services across the lifespan include: - Ages 0-5: Early Childhood Support – behavioral and developmental milestone assessments, parenting support, play therapy - Ages 6-12: Children – individual counseling, play therapy, family therapy - Ages 13-18: Adolescent – Teen counseling, substance use counseling, crisis intervention, sexual health counseling - Ages 18 and above – Individual therapy, couples counseling, psychiatric services, sexual health counseling, substance use counseling Fordland Clinic will leverage HRSA BHSE funds to increase access to behavioral health services through the expansion of mental health and addition of substance use disorder (SUD) services. Project activities include: - Leveraging community partnerships with organizations with a direct connection to underserved and underrepresented populations (racial/ethnic minorities, people who are uninsured, incarcerated individuals, etc.) - Further integrating primary care and behavioral health at each client encounter, conducting assessments and connecting clients with supportive services and scheduling follow-up appointments - Go beyond providing medical care and addressing transportation barriers within the community. - Expand the role of primary care providers in the use of FDA-approved medications for treatment of opioid use disorder through training and education. - Hiring additional behavioral health staff to expand access and meet the growing demand for services. - Contracting a psychiatrist to improve mental health outcomes and increase available services and access. - Hiring additional support staff to support in efficiency and the projected increase in patient volume. - Renovating current facilities to support increased behavioral and SUD provider and improve patient flow. - Increase provider training surrounding behavioral health and SUD services. Program goals to be achieved via the project activities listed above include: - Increase the number of patients receiving behavioral health services; and - Increase the number of patients receiving SUD services, including treatment with medications for opioid use disorder (MOUD)
