Ocean Ave, Suite 409 City & State: Boynton Beach, FL 33435 Genesis Community Health Center (Genesis) was formed in 2009 in response to a community needs assessment that identified Boynton Beach as an area of need, specifically due to the severe lack of access to primary care. This need was based upon evaluation of available services, a high incidence of poverty and uninsured individuals in the area, and a significant reliance on the emergency room for primary care. Genesis was formed specifically to meet the needs of its clients. As a Federally Qualified Health Center, Genesis has taken a leadership role in making care more accessible, working with partners and independently. Since officially opening its doors in 2010, Genesis Community Health has become the medical home for over 50,000 patients from Boynton Beach, Boca Raton and the surrounding areas. In 2023, Genesis Community Health served 5,400 unique patients, of which 22% were Hispanic, 42% were Black, and over 20% had incomes below 100% Federal Poverty Level. Additionally, Genesis Community Health is a Healthcare for the Homeless (HCH) provider, serving 97 patients in 2023, of which 32% were Black and 17% were Hispanic. Genesis Community Health offers an array of behavioral health services, for those suffering from Major Depression, Bipolar Disorder, Grief/Loss, trauma, or those needing substance abuse recovery and HIV/AIDS counseling. Genesis Community Health uses a holistic, strengths-based, empowerment approach to behavioral health treatment which explores the biological, social, and psychological needs and wants of every patient in care. Genesis is staffed by Licensed Clinical Social Workers, and other license behavioral health clinicians that provide on-site individual psychotherapy with the goal of helping persons whose behavioral health issues have impacted their ability to maintain their family, work, and personal lives. Genesis Community Health meets each patient’s unique needs by integrating behavioral health, medical, and dental services and given its FQHC status, no patient is ever turned away. Genesis Community Health will leverage HRSA BHSE funds to increase access to behavioral health services through the expansion of mental health and substance use disorder (SUD) services. Project activities include: - Leveraging community partnerships to better serve underserved populations (those involved in the criminal justice system, who are uninsured, who are homeless); - Integrating primary care, dental, and behavioral health, conducting assessments and connecting clients with Peer Support Specialists to expand access to supportive services; - Go beyond providing medical care by addressing social drivers of health through community collaborations (housing insecurity, food insecurity, financial strain); - Expand the role of primary care providers in the use of FDA-approved medications for treatment of opioid use disorder through training and education; - Hire additional behavioral health staff to expand access, meet the growing demand; - Broaden the use of mobile services across Genesis Community Health sites. Program goals to be achieved via the project activities listed above include: - Increase the number of patients receiving mental health services; and - Increase the number of patients receiving SUD services, including treatment with medications for opioid use disorder (MOUD)
