Request: Partnership Community Health Center is requesting ongoing federal funding through our service to expand behvaioral health services for residents of northeast Wisconsin in our scoped areas of Calumet, Outagamie, Waupaca, Winnebgo counties. 2024 Funding Request Year 1: $600,000 Year 2 $500,000 Brief description of the proposed project: Partnership Community Health Center (PCHC) will use BHSE funds to achieve two objectives: 1. Increase the number of patients receiving behavioral health services, and 2. Increase the number of patients receiving SUD services, including treatment with medications for opioid use disorder (MOUD) PCHC will expand access to substance use disorder (SUD) services and behavioral health care through personnel increases, extensive provider and team training, as well as clinical workflow and data collection improvements to ensure that all patients in need of services receive appropriate and timely care. In collaboration with local mental health leaders and multicultural coalitions, we will implement a Community Health Worker program that aims to decrease barriers to care and addresses the unmet need for behavioral health and SUD/MOUD services. We will focus on members of the African American, Hmong, Latino, refugee/newcomer communities, as well as those experiencing homelessness, as a priority of this work. Our focus will continue to meet the needs of our patients and community by expanding access to all services for low-income, uninsured and Medicaid recipients, as well as anyone in need of care. Targeted outreach and health insurance enrollment strategies will be a core component of our service delivery model and will ensure that patients remain connected to “coverage and care.” We will work to address the unique barriers faced in accessing treatment by racial and ethnic minorities, people who are uninsured or experiencing homelessness, and other underserved groups who experience higher rates of mental health conditions and SUDs. Our comprehensive approach to behavioral health services will enable more patients access treatment, recovery, and support services by addressing underlying social risk factors like food insecurity, unstable housing, lack of or limited health insurance, linguistic barriers, financial strain, and limited access to transportation. Service delivery sites and locations, and services to be provided: PCHC offers primary care, dental, and behavioral health care in Appleton and has two additional locations for dental care in Oshkosh and Waupaca. We continue to grow to meet the needs of our area’s underserved and low-income individuals, with the goal of improving both patient experience and health outcomes, as well as increasing our capacity to address patient needs within in a framework of health literacy, equity, cultural competency, and trauma-informed care. We will expand behavioral health services for mental health and substance use disorder.
