Incorporated as a non-profit organization with its federal tax exemption 501 (c) 3 since November 20,1986. CSPSP has four (4) sites located in Patillas, Maunabo, Salinas and Santa Isabel. Federal funds received are to serve Community Health Patients (CHC), MSFW and Public Housing Residents (PH). As part of our primary provision of care, a mental health service is provided on a limited basis. The lack of behavioral health providers is high (HPSA score 22) to fulfill the need of these services. This barrier could be addressed thru this grant. CSPSP is proposing a two (2) years project to expand behavioral health services and SUD; including children from twelve (12) years and older and adults with any mental health condition or opioid use disorder diagnosed. As per UDS 2023, CSPSP served 19,400 general patients, MSFW, Public housing, other. From this universe, 1,196 (6.16%) received some mental health service from ages 21 years and older; with 7.69 FTE; generating 3,495 visits on site and 127 virtual. Thru this funding of BHSE, we are proposing to achieve during the two project years, 500 new patients who will be receiving an appropriate mental health service. Also, we will be integrated the SUD services, including treatment with medications for opioid use disorder, to reach 100 new patients, at the end of project period. Main diagnosis attended as 2023 UDS data: depression, anxiety and mood disorders, alcohol and substance disorders, tobacco, attention deficit, behavior disorder, intimate violence and other mental disorders as drug dependence. This commitment is to expand behavioral health services in our four sites. Specific project goals will be set for the two years project: (1) Significantly expand the mental health and SUD services to serve new patients. (2) Provide an accessible mental health/ SUD service according to our target population. (3) Create a care management team with an individualized service plan for children, adolescent and adult served based on a comprehensive diagnosis. (4) Deliver culturally and linguistically competent services with special emphasis on ethnic and regional diversity for the underserved populations and (5) Achieve additional collaborative partners to strengthen our project, service plan and the development, evaluation and sustainability of continuous services and support in the overall system and the related transformation activities. We proposed 600 new patients to be served, with appropriate required visits annually. Our main goal is to integrate an adequate behavioral health staff including: 1.0 FTE Clinical licensed psychologist; .50 FTE licensed psychiatrist; 2.0 FTE Substance abuse counsellors; 1.0 FTE physician; 1.0 FTE Registered Nurse and a 1.0 BH coordinator; .40 FTE Pharmacist for a 6.90 FTE staff to be hired, in addition to the current mental health staff available.
