FY 2024 Behavioral Health Service Expansion - This BHSE initiative is proposing to significantly expand access to MOUD and SUD/OUD treatment as well as mental health counseling for these two target populations – the unsheltered population as well as youth and young adults accessing services at our Teen Clinic and three SBHCs. As there are no other substance use treatment programs in western Sonoma County anymore since the County has stopped providing these services, WCHC is in a unique position to expand access to MOUD and SUD/OUD treatment within a primary care setting in the local communities that we serve. As part of this BHSE initiative, WCHC will expand the role of Dr. Garrison-Jakel to become a 0.2 FTE Addiction Medicine Clinical Director that will be able to dedicate time towards securing trainings available through Sutter Health and connections to addiction medicine experts for our existing 14 providers so they receive the training and expertise needed to begin offering MOUD treatment to patients throughout our network of clinics. As the new Addiction Medicine Clinical Director, Dr. Garrison-Jakel will provide clinical supervision for our MAT Providers and family practice physicians, nurse practitioners, and psychiatric nurse practitioners as they receive training and begin providing MOUD treatment. In addition, WCHC will hire a 0.4 FTE Addiction Medicine Provider that will be located at the HCH Clinic under the direction of Dr. Garrison-Jakel to provide immediate increased access to MOUD treatment for the homeless population as well as any patients that is in need of MOUD / SUD services at WCHC. In addition, WCHC will expand access to mental health services by hiring 2.0 FTE Behavioral Health Specialists to provide much needed capacity to serve additional patients with one-on-one counseling, group visits, mental health assessments and intakes, as well as Family Systems Therapy. WCHC will hire a 1.0 FTE SBHC Community Health Worker to provide extensive linkages to social supports, track referrals and care coordination, assist with scheduling appointments, and ensure patients who are newly accessing mental health services and SUD/OUD treatment are being linked with food, transportation, housing, utility assistance, and other recovery-based and supportive services available within western Sonoma County. In addition, WCHC will hire a 1.0 FTE SBHC Family Case Manager that will provide a high level of case management for youth who present at the Teen Clinic and SBHCs as well as their families. The Family Case Manager will be highly trained to work with youth and students who are screened positive for a SUD/OUD or a mental health concern.
