Oliver (601)437-3050 jeo0705@yahoo.com Amount Assistance Requested: $600,000 Year 1 and $500,000 Year 2 The Claiborne County Family Health Center (CCFHC), a Federally Qualified Health Center (FQHC) (Grant Number H80CS00482), serves the residents of Claiborne, Copiah, Warren, and Jefferson counties in Mississippi through four service delivery sites. CCFHC's patient population faces significant economic challenges, with 44.2% living below 200% of the Federal Poverty Guideline (FPG). This project seeks to expand mental health and substance use disorder services in these communities, addressing critical gaps in care. Project Goals and Services Expansion: CCFHC is dedicated to enhancing its capacity to serve more patients and broaden the scope of its mental health and substance use disorder services through the following expansions: 1. Extended Provider Hours: Increased Patient Access: By extending the hours of operation for mental health providers, CCFHC will offer more appointment slots, thereby accommodating a greater number of patients and providing more flexibility for those with varying schedules. 2. Enhanced Counseling Services: • Comprehensive Care: Adding counseling services will address the psychological aspects of mental health conditions, complementing medication management and providing holistic care. • Improved Patient Outcomes: Evidence shows that integrating counseling with medication leads to better health outcomes, increasing patient satisfaction and retention. • Skill Development and Preventive Care: Counseling will equip patients with coping strategies, stress management techniques, and preventive care to build resilience and maintain mental wellness. • Increased Service Utilization: These services will attract more patients by offering a comprehensive suite of mental health care options, promoting personal growth, and improving overall quality of life. 3. Case Management Services: • Coordinated Care: Introducing case management services will facilitate the coordination of care for patients with complex needs, ensuring they receive timely and comprehensive treatment. • Expanded Reach: Case managers will proactively engage with the community through monthly outreach activities, increasing awareness and driving patient enrollment. • Prompt Treatment: Ensuring timely intervention for those identified with mental health disorders or substance use disorders will lead to better health outcomes and reduce the incidence of severe conditions. 4. Community Outreach and Education: • Raising Awareness: Monthly outreach programs will educate the community about the availability and benefits of mental health and substance use disorder services at CCFHC. • Increasing Patient Enrollment: Through targeted outreach and education initiatives, CCFHC aims to increase the number of patients served, particularly among underserved populations. 5. School-Based Services: • Accessible Care for Students: Mental health and substance use disorder services will be expanded at our three school-based sites, ensuring that students have convenient access to essential care. • Early Intervention: Providing these services in schools will facilitate early identification and intervention for mental health and substance use issues, promoting better long-term outcomes • Supportive Environment: School-based services will create a supportive environment, helping to reduce stigma and encourage utilization of mental health resources.
