FY 2024 Behavioral Health Service Expansion - Project Title: FY 24 Behavioral Health Services Expansion (HRSA-24-078) Applicant Organization Name: Davis Street Community Center, Inc Grant Number: H80CS28979 Project Director Name: Daniel Johnson, Chief Executive Officer/HRSA Project Director Brief Description of the Proposed Project: Davis Street Community Center, Inc., (DSCC) is a nonprofit, community-based health center located in San Leandro, California. The mission of DSCC is to provide a helping hand to those less fortunate, especially during times of crisis. To that end, we offer a range of primary and preventive care services, focusing on the most vulnerable and difficult-to-reach populations in Alameda County. Based on a recently completed needs assessment, more than 93% of DSCC’s service area residents have incomes at or below 200% of the Federal Poverty Guidelines (FPG) – a rate higher than the county, state, or nation – and more than four (4) in 10 residents (34.7%) are foreign-born, with most hailing from Mexico (11.6 %) and Philippines (4.4%). As illustrated in our 2023 Uniform Data System (UDS) report, DSCC provided mental health (MH) services to 295 patients via 1,733 visits (735 clinic visits and 544 virtual visits). While we had zero (0) substance use disorder (SUD) patients, we had one (1) patient who received medications for opioid use disorder (MOUD) disorder during the reporting period. DSCC is a trusted community medical home and therefore, is well positioned to address the gaps in BH access and care. Currently, our BH staff includes a psychiatrist, and a licensed clinical psychologist. Per our current Form 5A: Services Provided, DSCC already provides MH, SUD Services in Column I. Thus, a scope adjustment or change in scope to add the services will not be needed. DSCC is proposing to use funds for personnel to increase access to BH services, which will include the costs of hiring new staff to join our team. Furthermore, funds will be used for Alterations/Renovations (A/R) at DSCC’s single service delivery site. A/R activities include updating a space within our building to provide BH rooms whereas patients are currently seen via our medical exam rooms or telehealth. The proposed A/R project includes converting existing warehouse space into treatment offices for SUD, MOUD and MH. Our goal will be to add four new exam rooms, in addition we will convert existing administrative offices to MH offices. The updates, will modify and enhance the health center’s physical plant without changing its footprint to ensure that services are accessible, high quality, and ADA compliant. To increase the number of patients receiving MH services and SUD and MOUD services, DSCC will hire a 1.0 FTE Psyc Nurse Practitioner(MH/SUD); 1.0 FTE MFT/LCSW (MH/SUD), 0.70 FTE Medical Assistant; and 1.0 FTE Patient Navigator/Peer Specialist. . We will look to recruit providers and staff who have experience working with ethnic populations, with an emphasis on bilingual providers.
