CMSA was established in 1952 as a 501(c)(3) nonprofit to provide primary healthcare services to residents in frontier Carbon and Emery counties in Utah. Since 1952, CMSA expanded to provide affordable, needs-based, medical, dental, behavioral health, SUD, and pharmaceutical services at sites located in Sunnyside, Helper, and Price, Utah. CMSA’s 4,000+ patients originate from 13 zip code tabulation areas—88% from Carbon and 8% from Emery counties. Carbon and Emery counties comprise 7% of Utah’s landmass, but only 1% of Utah’s population—equating to approximately five people per square mile. CMSA will expand mental health services to at least 300 new patients through an integrated care model by hiring 1.75 FTE other licensed mental health staff and a .5 FTE case manager/peer support serving all CMSA sites. CMSA will also provide SUD treatment and counseling services at all CMSA sites by hiring a .25 FTE other licensed mental health staff supporting SUD and alcohol use disorder treatment services to at least 50 new SUD patients. CMSA through an integrated behavioral health model will serve at least 150 new Medications for Opioid Use Disorders (MOUD) patients. All CMSA providers are trained and licensed to deliver medications for opioid use disorder (MOUD) as part of their scope of practice. Continuing medical education ensures our team remains up to date with the latest practices and evidence-based protocols from esteemed organizations like the American Society of Addiction Medicine (ASAM) and the Substance Abuse and Mental Health Services Administration (SAMHSA). This robust foundation equips our providers with the knowledge and skills needed to offer effective, evidence-based treatment to our patients. At present, CMSA documented that 100% of patients with an identified OUD have been offered MOUD services, with 78% receiving treatment within a month of diagnosis. CMSA care delivery is structured around our Patient Centered Medical Home, and we are Joint Commission certified in Ambulatory Care, PCMH and Behavioral Health. Our care teams provide patient case management services to access counseling, specialty referrals, and managing the follow-up needs for the individual and family members in pursuing care plan objectives. Staff inform patients and conduct outreach on the range of services available on a sliding fee. CMSA partners with Four Corners Mental Health, Utah Support Advocates for Recovery Awareness (USARA), Life Balance, and Castleview Hospital for more intensive services such as detoxification, residential treatment, medical assessments, and Intensive Outpatient programs, as well as working with Medicaid patients for Mental Health and inpatient SUD treatments in our region. CMSA will invest the BHSE grant funds in expanding behavioral health, SUD, and MOUD treatment services through an integrated care model in frontier communities and CMSA included support for minor renovations, increased direct patient hours, telehealth, and coordination with community partners.
