PrimaryPlus is a Federally Qualified Health Center that offers the full continuum of care to socio-economically disadvantaged Appalachian and medically indigent populations. Primary Plus locations offer comprehensive health services including primary and pediatric care, women’s health, dental care, pharmacy and medication assistance, behavioral health including both child and adult psychology, substance use disorder services and specialty care such as pulmonology, dermatology, endocrinology, neurology, and sports medicine. The commitment of PrimaryPlus in providing healthcare to their service area communities is unparalleled. PrimaryPlus offers comprehensive behavioral health services and SUD services including medications for opioid use disorder (MOUD) to support individuals and their families at every stage of life. PrimaryPlus tailers services to meet the needs of specific different age groups, offering specific developmental services. PrimaryPlus offers behavioral health and SUD services at all clinic locations, as reflected in Form 5B. Specialized behavioral health and SUD services across the lifespan include the following: - Ages 0-5: Early Childhood Support – behavioral and developmental milestone assessments, parenting support, play therapy - Ages 6-12: Children – individual counseling, play therapy, family therapy - Ages 13-18: Adolescent – Teen counseling, substance use counseling, crisis intervention, sexual health counseling - Ages 18 and above – Individual therapy, couples counseling, psychiatric services, sexual health counseling, substance use counseling, treatment with MOUD PrimaryPlus will leverage HRSA BHSE funds to increase access to behavioral health services through the expansion of mental health and substance use disorder (SUD) services including MOUD. Project activities include: - Leveraging community partnerships with organizations with a direct connection to underserved and underrepresented populations. - Leverage relationship with service area EMS and coroner offices to identify and perform targeted outside in high-risk communities. - Further integrating primary care and behavioral health at each client encounter, conducting assessments and connecting clients with supportive services and scheduling follow-up appointments. - Go beyond providing medical care and addressing the social drivers of health through community collaborations. - Expand the role of primary care providers in the use of FDA-approved medications for treatment of opioid use disorder through training and education. - Train current staff to treat SUD through MOUD. - Hiring a Psychiatric Nurse Practitioner and Licensed Clinical Social Worker (LCSW) to provide MOUD. - Hire additional behavioral health staff to meet growing behavioral health and SUD needs. - Hire a Project Manager to oversee the project and ensure project success. - Hire a Clinical Psychologist to perform therapy and group sessions. - Utilize the mobile unit to take SUD and behavioral health services directly to the at-risk patient population. - Broadening use of telehealth across PrimaryPlus sites to enhance access to behavioral health services. Program goals to be achieved via the project activities listed above include: - Increase the number of patients receiving behavioral health services; and - Increase the number of patients receiving SUD services, including treatment with MOUD.
