FY 2024 Behavioral Health Service Expansion - Salina Health Education Foundation dba Salina Family Healthcare Center 651 E. Prescott Ave., Salina, Kansas 67401 H80CS04216 Salina Health Education Foundation, Inc. (SHEF) was founded in 1979 as a community based, non-profit family medicine residency program. Salina Cares, a free clinic to serve the low income uninsured, was founded in 1991. Providers from SHEF began staffing Salina Cares in 1998. The organizations merged in 2004 to form Salina Family Healthcare Center (SFHC), which became a Federally Qualified Health Center on December 1, 2004. In January 2013, SFHC achieved recognition as a Level 3 Patient-Centered Medical Home™ and has successfully maintained certification in the years since. Saline County is home to 53,596 people, of which the majority of residents are white (48,236). The target population is the Saline County residents living below 200% of the Federal Poverty Level (17,151). According to Mental Health America’s 2023 State of Mental Health in America, Kansas’ Adult Ranking was 51st out of 51, which includes all 50 states and the District of Columbia. Measures that made up the Adult Ranking included: Adults with any Mental Illness (AMI), Adults with Substance Use Disorder in the Past Year, Adults with Serious Thoughts of Suicide, Adults with AMI who Did Not Receive Treatment, Adults with AMI Reporting Unmet Need, Adults with AMI who are Uninsured, and Adults Reporting 14+ Mentally Unhealthy Days a Month who Could Not See a Doctor Due to Costs. By comparing the Service Area of Saline County to Kansas data, further evidence of need is demonstrated in that all of the following indicators exceed the Kansas rate: the percentage of adults who binge drink, alcohol-impaired driving deaths, the death rate due to drug poisoning, liquor store density, percent of Medicare Part D Beneficiaries having daily opioid dosage >= 50 Morphine Milligram Equivalents (MME), percent Medicare Part D Beneficiaries receiving both long and short-acting opioids, average monthly Women, Infants, and Children (WIC) participation, health insurance spending-to-income ratio, Kansas Medical Assistance programs for adults and children, age-adjusted years of potential life lost-suicide, depression-Medicare population, mental behavior hospital admissions rate, and persons with a cognitive difficulty. The proposed project will address these needs by increasing SFHC’s behavioral health staff, strengthening its link to the local Certified Community Behavioral Health Clinic, and extending its reach into unserved populations, including the homeless and young adult populations. Most services will be provided in-person at Salina Family Healthcare Center’s main site at 651 East Prescott Ave., in Salina, Kansas, and a few will be provided via telehealth. In addition, SFHC’s community outreach staff, which operates the organization’s two mobile medical units throughout Saline County and offers health screenings, education classes, immunizations, care coordination, and provider visits, will serve as a referral source back to the clinic for mental health and substance use disorder treatment. Behavioral staff to be hired include 2.0 FTEs Behaviorists and a 1.0 FTE RN Care Coordinator. Together, this team will increase the number of new patients who receive mental health services by 350, increase the number of new patients who receive substance use disorder services by 50, and increase the number of new patients who receive treatment with MOUD by five (5). This will result in a total of 150 new, unduplicated patients receiving behavioral health services at SFHC.
