FY 2024 Behavioral Health Service Expansion - Project Title: FY 24 Behavioral Health Services Expansion (HRSA-24-078) Applicant Organization Name: Community Health Center of Lubbock, Inc. Grant Number: H80CS00766 Project Director Name: Michael Sullivan The Community Health Center of Lubbock, Inc. (CHCL) is a multi-service health care organization and trusted Federally Qualified Health Center (FQHC) located in Lubbock County, Texas. Founded in 1992 and obtaining FQHC designation one year later, CHCL is the largest community health care delivery system in Lubbock County serving approximately 20,000 underserved individuals and families each year. Today, CHCL operates out of 10 fixed service delivery sites and two mobile units, providing medical, dental, behavioral health and vision services to nearly all of Lubbock County. Additionally, with support from two mobile units, CHCL also serves the four surrounding counties of Crosby, Garza, Lynn, and Terry. CHCL’s service area spans 4,700 square miles and consists of approximately 324,335 service area residents whereby more than one-third (37.4%) are part of CHCL’s target population, individuals at or below 200% of the Federal Poverty Guidelines (FPG) (2022 American Community Survey, 5-Year Estimates). Through its mission, CHCL provides quality, affordable healthcare while minimizing barriers to care throughout the South Plains area of the Texas Panhandle. Through the FY24 Behavioral Health Services Expansion (BHSE) initiative, CHCL will increase the number of patients receiving mental health (MH) services and increase the number of patients receiving substance use disorder (SUD) services, including patients receiving medications for opioid use disorder (MOUD). We will focus our efforts on serving low-income, uninsured adults and children with a behavioral health (BH) diagnosis. With funding, CHCL will be able to deliver a full continuum of BH services at its main clinic located at 1610 5th Street, and at its 96 West clinic located at 2401 Fulton Avenue #B, which are both fixed service delivery sites. With funding, CHCL proposes personnel (new staff), education and training opportunities, recruitment and incentive opportunities, as well as tablets to support patient screenings for MH, SUD, and MOUD services across all CHCL sites and services/programs. This process will integrate with CHCL’s Electronic Health Record, EPIC, and will allow for assessments to be completed in a variety of settings, automatically be added to the patient’s medical record, and for positive screenings to be flagged for follow-up. CHCL will also expand its scope of work with Integrated Health Centers, Inc. (IHC) who currently provide Psychiatry Services for CHCL patients to include SUD and MOUD services. As illustrated in our 2023 Uniform Data System (UDS) report, CHCL provided MH services to 666 patients via 3,959 visits (3,281 clinic visits and 678 virtual visits). Currently, we refer patients in need of SUD services to Grace Manor through a Memorandum of Understanding (MOU). Thus, we reported zero (0) SUD patients and MOUD patients during the reporting period. CHCL is a trusted community medical home and therefore, is well positioned to address the gaps in BH access and care. Currently, our BH staff include Licensed Professional Counselors and Nurse Practitioners. Per our current Form 5A: Services Provided, CHCL provides Health Care for the Homeless (HCH) Required SUD Services in Column I and Column III (through a Memorandum of Understanding (MOU) with Grace Manor). MH Services are currently provided in Column I and Column III (through an MOU with StarCare Specialty Health System), and Psychiatry Services are provided in Column II (through an agreement with IHC). Thus, upon award, CHCL will submit a Scope Adjustment application to add HCH Required SUD to Column II on Form 5A to reflect the expanded scope with IHC.
