FY 2024 Behavioral Health Service Expansion - Greater Family Health (H80CS00288) is a community-based and patient-directed provider of integrated primary health care services. The organization is recognized as a Federally Qualified Health Center (since 2002), is accredited by The Joint Commission (since 2007), and is certified as a Primary Care Medical Home (since 2013). The organization’s mission is to provide quality, affordable health care for all, including those without the ability to pay. Greater Family Health proposes to use FY24 Behavioral Health Service Expansion funding to increase the number of patients receiving mental health services and increase the number of patients receiving SUD services, including patients receiving treatment with MOUD, by doing the following: 1) Hiring additional LCSWs to provide mental health services at 6 of our health centers. 2) Hiring a Peer Recovery Support Specialist to provide individual and group support services for patients in our Substance Use Disorder program. 3) Increase the number of our Family Medicine Licensed Independent Practitioners performing MAT intake. 4) Adding more Psychiatry capacity. The primary need of the target population is access to quality, well-integrated primary health care, including behavioral health care, which is particularly limited for low-income people. According to UDS Mapper, of the 1,349,360 residents in the service area, 13.44% are covered by Medicaid while 7.24% (about 180,400) lack any health insurance. The percentage of adults who have delayed or not sought care due to cost is estimated to be 16.66% and the percentage of adults with no usual source of health care is estimated to be 24.22%.
