Kadar, Chief Executive Officer Brief description of your organization: Founded in 1995, Pomona Community Health Center dba ParkTree Community Health Center (ParkTree), is committed to its mission “to be the medical home for the underserved in our community by providing high quality preventive and primary care health services.” ParkTree is a Section 330(e) grantee, operating five (5) service delivery sites (3-Los Angeles County, 2-San Bernardino County). ParkTree provides comprehensive and integrative primary health care to people of all ages. Our services include primary care for adults and children, specialty services for optometry, podiatry and chiropractic medicine, behavioral health (depression, anxiety, substance use), and oral health (diagnostic, preventative, restorative). ParkTree has health educators, case managers and a call center that goes the extra mile to answer the calls and questions of our patients and families. ParkTree offers health insurance enrollment services for our patients and members of the community. ParkTree accepts several types of health insurance including Medicaid, Medicare, Covered California and commercial plans. And for those without insurance, we offer a sliding fee discount program. ParkTree never turns anyone away due to their inability to pay. Brief summary of how your proposal for increasing hours of operation will meet identified needs in your community: ParkTree is committed to expanding the hours of operations at three health centers in our scope of project (currently listed on Form 5B) to meet the needs of our community. ParkTree will add ~20 hours in total to our service sites which provide primary medical, dental and behavioral health services. ParkTree has the resources, capabilities and collaborations in place necessary to implement this HRSA-25-084 Bureau of Primary Health Care Expanded Hours funding opportunity. The basic needs to be addressed will be to expand our comprehensive services on days and at times most in demand and requested by our patients and community. ParkTree will add hours in the early morning and during the evening thereby increasing availability for children and their parents and those who work during routine daytime hours. These added hours will assist families gain access to healthcare who otherwise may have had to utilize urgent care or emergency room services due to lack of access of appointments after school or work. This program will allow for increased access for people of all ages in both primary prevention and chronic disease management. Approximately 3,060 new appointments will be created through this program. The H8J funds will be used to hire clinical care team members who will provide direct (medical/dental assistants, case managers, clinicians) and indirect (referral, enrollment staff) care services to support the visits that will occur during the extended hours. These clinical care team members will support the patient and family in the Patient Centered Medical Home model by performing duties before, during and after the visit.
