Austin Community Health Center, Inc., dba Community Health Network (CHN) proposes increasing the number of patients screened for cervical, breast and colon cancer from the current UDS 2021 Report baseline percent compliance to the 2023 goal for cervical cancer screening (26.7%) to 25.0%; breast cancer screening (16.6%) to 40.0%; and colon cancer screening (7.5%) to 20.0%. The improvement will be achieved in partnership with the NCI-designated MD Anderson Cancer Hospital in Houston, Texas providing training and oversight of CHN’s Patient Navigators and Outreach staff. Additional collaboration includes provider education through biweekly ECHO sessions and will provide CHN with no cost FIT cards. MD Anderson will also assist uninsured individuals with an abnormal colon cancer screen to obtain follow-up colonoscopy procedures. Patient Navigation, screenings with referral tracking and follow-up of abnormal test results, outreach in the community and workforce development are key activities that will drive the percent compliance towards goal for CHN’s service area and the patients served by 2023. There is a large minority population residing in CHN’s service area of Brazoria, Galveston and Harris Counties, and many face barriers accessing preventive screening services. The American Community Survey 2015-2019 5-year Estimate demonstrates a population estimate of 360,677 for Brazoria County. Ethnically, 31.6% of the population is Hispanic; racially 14.8% of the population is African American, 72.9% is white (non-Hispanic), 7.2% is Asian and 4.6 % is other or multiracial. 50.3% of the county is male and 49.7 % is female. An estimate of 332,885 reside in Galveston County. Ethnically, 25.4% of the population is Hispanic; racially 12.5% of the population is African American, 79.5% is white (non-Hispanic), 3.4% is Asian and 7.6% other or multiracial. 48.9% of the county is male and 51.1% is female.1An estimated 54,751people live within the 77089 Harris County service area. Ethnically, 49.0% of the population is Hispanic; racially 16.0% is African American, 56.8% is white (non-Hispanic), 15.4% is Asian and 8.8% other and multiracial. 50.1 of the county is male and 49.9% is female. Individuals face transportation issues due to the absence of a public transportation system connecting Brazoria County’s Northern and Southern ends and connecting the three Counties together. There are large areas within Brazoria County that are considered rural; Galveston County includes areas designated as rural; and both counties include medically underserved areas and health professional shortage areas. Both counties also include areas with no broadband access. The data reflects that Adults are uninsured at higher rates than children with Harris County adult residents at 27% compared to Brazoria at 19% and Galveston County at 20% verses Texas at 24.
