(MMA), a community-based 501(c)3 non-profit and a Federally Qualified Health Center Look-Alike (FQHC LAL), has been providing care to residents in Monticello, Kentucky for nearly 50 years. Beginning in 1975 as an independent physician's office, MMA received its FQHC LAL designation in March 2021 and now runs a proficient team of 5 physicians, 9 nurse practitioners, and over 45 dedicated support staff. MMA is recognized as a Patient Centered Medical Home (PCMH) by the National Committee for Quality Assurance and has been ranked in the top 5 of independently owned rural health clinics (RHCs) in the Southeast prior to achieving FQHC LAL status. Striving to serve all patients, MMA achieves their mission to “maintain the highest standard of medical care while serving vulnerable and underserved communities in Wayne County, Kentucky.” Today, MMA provides care to residents located in southern Kentucky within Monticello (ZCTA 42633), covering an estimated population of 18,004 through two locations, a service site and a pharmacy. The service area for MMA includes Monticello at 42633 with a population of 18,004. MMA serves vulnerable, minoritized populations that are greatly impacted by social determinants of health. The MMA patient demographic consists of low-income patients (52.77% living below 200% of the FPL) and minority groups. MMA patients are vulnerable to a host of health problems tied to the social determinants of health that impact poor health care outcomes, including poor physical and mental health. MMA is applying for HRSA’s New Access Point funding to better serve populations of focus within the service area for its proposed, full-time Mobile Unit site, which will be located at 1215 N. Main St Monticello, KY 42633. This site will take primary care, behavioral health services to those who need it most within the service area and allow MMA to expand its service area (ZCTA 42633, 42533, 42518, 42558, 42564) and reach. Grant funds will allow MMA to provide behavioral health and substance use disorder (SUD) services. MMA proposes to serve 12,000 unduplicated patients by fiscal year 2026.
