Chavez Ave., Suite 6100 Project Director Name: Grace Floutsis, M.D., CEO/CMO Contact Phone Number: 323-859-3604 Email Address: floutsg@ah.org Website: https://wmchealthcenter.org/ Grant Request Amount and Type: CHC; $650,000 Total Number of Unduplicated Patients Projected to be Served in CY 2026: 29,475 Brief Description of the Proposed Project: White Memorial Community Health Center (WMCHC) was incorporated in 2014 as a 501(c)(3) nonprofit community clinic, commenced operations in May 2017, and was designated as a Federally Qualified Health Center (FQHC) Look-Alike in April 2018. WMCHC was recognized by the National Committee for Quality Assurance as a Patient-Centered Medical Home (PCMH) in April 2020. WMCHC now serves nearly 16,000 children/adolescents and 6,000 adults who primarily reside in the low-income Los Angeles communities of Boyle Heights, Maywood, Bell Gardens, Bell, Huntington Park, South Gate, and adjacent neighborhoods. Growth has been significant and continuous, and there is great need to build additional capacity for increased access to comprehensive health services. With the support of FY 2025 New Access Point funding, WMCHC will add a new permanent site located at 1701 E. Cesar E. Chavez Ave., Suite 456, Los Angeles, CA 90033, which will join our four existing health center delivery sites at 1828 E. Cesar E. Chavez Ave., Suite 4300, Los Angeles, CA 90033; 1828 E. Cesar E. Chavez Ave., Suite 4100, Los Angeles, CA 90033; 1828 E. Cesar E. Chavez Ave., Suite 5000, Los Angeles, CA 90033; and 1200 Plaza Del Sol E, Los Angeles, CA 90033. Needs to be Addressed: Recent community health needs assessments have strongly indicated a need for greater access to comprehensive primary and preventive health care, including medical, dental, behavioral health, enabling, and specialty care services. Additionally, chronic health conditions persist in the service area at elevated levels relative to Los Angeles County and California as a whole, including high rates of heart disease and hypertension, stroke, chronic liver disease and cirrhosis, late prenatal care, obesity, and diabetes – all of which disproportionately affect the health of low-income, underserved residents. WMCHC is well versed in providing culturally and linguistically appropriate health center services, and will continue to address identified health care needs and barriers in our service area by providing a continuum of care that enables patients to seamlessly access comprehensive services without experiencing disconnection from their medical home. Proposed Services and Population Groups to be Served: The new permanent site will be co-located with three of WMCHC’s existing clinics in a separate building on the Adventist Health White Memorial hospital campus, and approximately one mile from our school-based health center. Collectively, our proposed NAP project will significantly expand access to primary and preventive care services for low-income service area residents. Consistent with our PCMH service delivery model, our health center will continue to utilize a “whole person approach” that includes integrated primary care and supporting services that address social determinants of health. WMCHC offers a full range of health care services across the lifecycle, including, but not limited to, integrated primary medical, dental, and behavioral health care, women’s health, well child services, and enabling services, such as health education, care management, and eligibility and enrollment assistance. In compliance with FQHC requirements, WMCHC’s target population will remain individuals and families living at or below 200% of the Federal Poverty Guidelines. All services will be provided in-house or through formal referral arrangements in compliance with WMCHC’s board-approved sliding fee discount schedule.
