CLCHC provides comprehensive health and enabling supporting services for patients across the lifecycle. CLCHC currently operates seven (7) service delivery sites located at: 700 W. 19th Street, Suites A-B, Costa Mesa - and a Mobile Unit linked to this address; 15785 Laguna Canyon Road, Suite 250, Irvine; 1444 S. Main Street, Santa Ana; 27800 Medical Center Road, Suite 108; Mission Viejo; 27800 Medical Center Road, Suite 109, Mission Viejo; and 27800 Medical Center Road, Suite 110, Mission Viejo. This network is supported by three (3) administrative sites located at: 27271 Las Ramblas, Suite 250, Mission Viejo; 27271 Las Ramblas, Suite 350, Mission Viejo; and 18201 McDurmott West, Suite E, Irvine. In its New Access Point Application, CLCHC proposes to add a mobile unit, to support its Health Care for the Homeless designation; serving the City of Santa Ana. Needs to be Addressed: The service area for Celebrating Life Community Health Center (CLCHC) covers 20 geographical ZIP Codes in Orange County, California. Within the service area, 1,851 individuals identify as homeless – a key target population of CLCHC. Referencing the Health Center Program GeoCare Navigator, within the proposed service area the proposed service area identifies a Population Total of 980,913, of which 213,811 identify as low-income, and 77,040 as uninsured. The data tables also noted 164,397 individuals insured by Medicaid. Current Health Center Program participants, served 7.03% of total service area residents. The total penetration of low-income residents served by the Health Center Program was 32.27%. These data indicate: 144,816 low-income residents; 63,739 uninsured residents; and 117,892 Medicaid/Public Insured residents, unserved by the Health Center Program. Nearly two-thirds of low-income service area residents, 62.5%, are “non-White” or persons of color. More than one-third (38.6%) are Hispanic or Latino, and almost the same amount (37.5%) is Non-Hispanic Caucasian (European plus West Asian and North African, categorized by the U.S. Census Burau as “White”). Nearly one in five (18.5%) are Asian. Proposed Services and Population Groups to be Served: CLCHC’s populations of focus in keeping with FQHC requirements, CLCHC is focused on serving the homeless, low-income, uninsured, underserved, and the Medicaid, Medicare, and other publicly insured individuals within its proposed service area. CLCHC proposes to provide all Form 5A: Required Services, to include an array of additional health care services, including, but not limited to, general primary care, women’s health, well child services, mental health, substance use disorder, and enabling services, such as care management and insurance enrollment services.
