Health Center Program - McDermott Center, doing business as Haymarket Center, is Chicago’s premier integrated service system for individuals suffering from untreated medical and behavioral health conditions and homelessness. Haymarket is Chicago’s only FQHC LAL operating its own co-located inpatient treatment programs for behavioral health conditions. If awarded, Haymarket Center’s New Access Points (NAP) project will leverage the requested $650,000 in Healthcare for the Homeless funding to expand Health Center services to 3,500 new patients – rising from a budgeted capacity of 1,500 a year to 5,000 patients during the 2026 program year who are suffering from untreated medical and behavioral health conditions, are mostly experiencing homelessness in Chicago’s most underserved neighborhoods, and are likely to otherwise receive care in emergency departments rather than in integrated medical homes. Haymarket’s NAP project will make medical providers available to individuals on weeknights and Saturdays. Patients will be able to receive services 24 hours a day, 365 days of the year – regardless of the inability to pay. The dominant Health Center Program in the zip code in which Haymarket is located, Heartland Health Outreach, is closing its doors. Heartland will cease all its Health Center Program services and no longer serve anyone with the site’s Healthcare for the Homeless grant funding. Haymarket will use its NAP project to promptly fill this profound service gap. Haymarket will use leveraged outreach and transportation services in order to connect persons experiencing unsheltered homelessness to our NAP sites. Chicago’s homeless population has increased in recent years. Most patients will come from our service area comprising 18 contiguous Chicago zip codes with an Unmet Needs Score of 77.42. Haymarket’s NAP Project systematically meets the needs of the patient population. To address high rates of untreated medical conditions, medical risk factors inflicted by health inequities, and the tendency to receive care in emergency departments rather than in primary care physicians’ offices, Haymarket’s NAP project will provide, directly or through partnership, general primary medical care, diagnostic laboratory and radiology services, screenings for health conditions, voluntary family planning, immunizations, health education, well child services, pharmaceutical services (often from an onsite discount pharmacy), preventive dental, gynecological services, obstetrical care, and referrals to specialty care. There will be constant coverage for emergencies during and after hours. To address behavioral health conditions among the patient population, Haymarket’s NAP project will provide inpatient and outpatient SUD treatment, medically monitored withdrawal management, psychiatric care, and psycho-behavioral treatment for co-occurring mental health disorders. To address needs related to detrimental social determinants of health, Haymarket’s NAP project will provide case management, translation services, transportation, assistance accessing public benefits such as Medicaid, and employment services. To address the population’s urgent need for housing, the NAP project will deliver case management to secure stable housing. This is critical because homelessness is itself a barrier to favorable outcomes across health domains. Haymarket’s NAP Project includes all three sites in its existing Look-alike scope of project: 108 N Sangamon Street (floors 3-6), 120 N Sangamon Street, and 124 N Sangamon Street, all in Chicago, IL 60607-2202. Haymarket’s extensive outreach to populations experiencing homelessness in encampments and public settings, using leveraged resources, will ensure the NAP project will reach those who need it the most.
