Health Center Program - Service Area Competition ID: 247, Sarasota, FL Applicant Name: CenterPlace Health Health Center Award Number: H80CS31892 Unduplicated Patients to be served in 2026: 29,960 HRSA Funding Request: $2,332,057 CenterPlace Health (CPH) is a not-for-profit 501(c)(3) Section 330(e) Community Health Center grantee established in 2008 in Sarasota County, Florida. CPH serves as the primary community safety-net organization that delivers quality medical, dental chiropractic and behavioral healthcare to children, adults and families in need in Sarasota County regardless of their insurance status or ability to pay. Needs: In the past six years, the number of individuals in our care has grown from nearly 7,530 in 2018 to over 28,000 in 2023. The significant increase in the number of patients is due not only to the overall population growth in the area, but also, to a surge in the number of uninsured individuals seeking our services. Our patient target for 2026 includes an estimated 7% increase, consistent with our average growth rate prior to 2023 when we acquired two new sites. Health disparities impact underserved groups such as Black/African American and Hispanic/Latino residents who experience premature death from diabetes, heart disease, cancer, unintentional injuries, behavioral and substance use issues, along with a host of health issues which negatively impact birth outcomes. In addition, our county’s rates of depression, suicide deaths, non-fatal intentional self-harm injuries, and opioid overdose deaths along with a number of other indicators of mental health and substance use challenges outpace state averages.?Population Groups: Our core service area is home to 373,204 persons; our target population includes 93,301 individuals, i.e., those with incomes below the federal poverty guidelines (FPG) and those who are uninsured, underinsured, vulnerable and otherwise medically underserved populations (MUP).CPH cares for individuals of all ages, a significant proportion of whom represent traditionally underserved and marginalized groups. For example, individuals who are homeless comprise a significantly higher proportion of our patient population (2.8%) compared to their representation in the general county (0.09%) or state population (0.013%). Blacks or African Americans (non-Hispanic) are also represented at a higher rate (11.9%) compared to the general county population (4.7%). Similarly, Hispanic or Latino patients comprise 34.5% of our patient population compared to 10.3% in the county and 27.1% in the state’s general population. More than half of our patients (51%) are non-white and 22.7% speak a primary language other than English.? More than 70% are either uninsured or enrolled in Medicaid. Forty-nine percent are at or below 100% of the federal poverty level (FPL) and 18% fall between 101% and 200% of the FPL. Proposed Services: CPH provides all four service types required under this NOFO for the Sarasota, Florida area to include Medical, Dental, Mental Health, and Enabling services. Our teams deliver culturally competent primary care, OB/GYN, chiropractic, dental, lab, pharmacy and behavioral health services at seven fixed sites throughout Sarasota County. Our behavioral health program has grown significantly in the past two years with the addition of in-house psychiatric providers and therapists to meet the growing demand for critical mental health services. Virtual visits continue to be popular among our patients, facilitating access to care for individuals facing transportation barriers and other constraints. CPH works closely with community partners to address unmet needs and ensure linkages to care and to community resources. Our teams are actively engaged in outreach and community health assessment efforts. Our case managers, community health workers, and financial counselors work with patients to coordinate care, assist with insurance enrollment, and address social determinants of health.
