Health Center Program - Background: Kagman Community Health Center (KCHC) is located on the islands of Saipan and Tinian, population 45,429, part of the Commonwealth of the Northern Mariana Islands (CNMI) that was formed in 1978 and was formerly a trust territory of the United Nations. The CNMI is located about 3,700 miles west of Hawaii a remote area expanse of the Pacific Ocean. KCHC has three clinic locations that provide general primary medical care, preventative, and behavioral health services in the villages of Kagman and San Antonio on Saipan and the village of San Jose on Tinian. KCHC is the HRSA 330 grantee of record for H80CS31624, Service Area ID 276, Saipan, MP. Established in 2012, KCHC continues to provide general primary medical care and preventive health services to Saipan residents with the incorporation of behavioral health counseling services in 2019. In 2020, general primary medical care, preventative, and behavioral health services were expanded to Tinian during the COVID-19 pandemic through a new access point funding award. In 2023 a newly established clinic in the village of San Antonio was opened to expand care services to the southern region of the island. All three clinics are staffed with dedicated and experienced local healthcare professionals and support staff that serve their community. The KCHC Board of Directors governing the health center are composed of committed and passionate community members who themselves are regular patients of the health center. The community representation of the Board contributes to and enhances the health center’s patient centered approach to care. Service Area Needs/Target Population: KCHC’s purpose focuses on providing quality and equitable general primary medical care, preventative, and behavioral health services to residents of Saipan and Tinian through enhanced access and affordable care. The KCHC serves areas with a high proportion of chronic illnesses, low-income families, and uninsured populations. Additionally, it serves a diverse group of races and ethnicities with roughly one third indigenous Chamorro and Carolinian, and one third Filipino. In 2023, the KCHC unduplicated patient population consisted of the following: 28.32% uninsured, 42.23% Medicaid, 25.58% private insurance, and 3.87% Medicare. The most common diseases seen by KCHC patients are hypertension, type 2 diabetes, and high cholesterol. Based on the needs of the community served, KCHC continues to focus its clinical efforts on non-communicable chronic disease care management and cancer screening. Most importantly, KCHC had several clinical improvements on its patient outcomes from 2022 to the 2nd quarter of 2024 that includes: 1) Increased HIV screening - 24% to 77%, 2) Increased hypertension control - 43% to 80%, and 3) Increased PAP screening - 47% to 65%. On-going assessments are conducted to ensure programs and initiatives align with the needs of the community. Such initiatives include the incorporation of clinical programs to address the needs of at-risk and/or high-risk populations. Furthermore, KCHC aims to continue to meet the needs of the community through on-going patient and community assessments and the development of new and/or advanced programs. Proposed Services: KCHC ensures the continuity and accessibility of care on Saipan and Tinian through the provision of general primary medical care, preventative, and behavioral health services. Additionally, KCHC meets a critical community need by providing affordable care to underserved and largely populated areas in the CNMI. With a strong community board, ongoing alignment with community needs, growth of programs, and focus on providing care to the most vulnerable populations, KCHC continues to fulfill its mission in the community. KCHC projects to serve 4,000 unduplicated patients in the calendar year 2026.
