Health Center Program - GRANT #: H80CS29030 SAC ID# 395 HISTORY/SERVICE AREA/TARGET POPULATION: Grant County Public Hospital District No. 5 (GCPHD) is a Federally Qualified Health Center located in rural central Washington State. The hospital district is a public entity organized as a municipal corporation in the State of Washington for the purpose of providing health care services to the residents of the district and other persons served by the district. GCPHD was formed in November 1984 by a vote of the people when the voters recognized and responded to the growing population in and around the Mattawa area, and their need for direct access to primary care services. GCPHD established the Mattawa Community Medical Clinic (MCMC) to meet the local demand. The service area is defined as the hospital district boundaries, identified by two primary zip codes: 99349 and 99321 and a portion of 99357. MCMC provides a health care home for all people living in the area, with a focus on people with income below 200% of the Federal Poverty Level (FPL), the uninsured, and underinsured (includes people with Medicaid and Medicare). MAJOR HEALTH CARE NEEDS / BARRIERS TO CARE: Major health barriers for the target population include high rates of poverty – 43% of the residents live at or below 200% FPL, and 99% of the public school students are eligible for free or reduced school lunch. The service area population exceeds severe national benchmarks for obesity, hypertension, teen births, poor childhood immunizations, and drug overdoses. MCMC provides a full range of family practice primary care services in a patient-centered medical home: prevention, diagnosis and treatment of acute and chronic illness; chronic disease management; and referrals and follow-up for specialty diagnostic treatment. Prevention includes well-child exams, cancer screenings, immunizations, family planning, WIC and comprehensive dental services. MCMC also provides comprehensive dental and behavioral health services. All health services are provided directly by program staff or contract, without regard for the ability of patients to pay for care. Service Area Competition funding will allow MCMC to continue to operate four clinic sites in Mattawa and a mobile clinic. In 2026, MCMC will serve 2,836 unduplicated patients.
