(SJH) is a 501(c)(3) nonprofit organization and Federally Qualified Health Center (FQHC) that opened in 2016 in Newark, NJ with New Access Points (NAP) funding. Its mission is to provide full spectrum quality primary care in a culturally competent setting regardless of ability to pay. SJH’s target population is low-income individuals living at or below 200 percent of the Federal Poverty Level (FPL) within SJH’s service area in the city of Newark and its surrounding communities in Essex County, Union County, and Hudson County, NJ. SJH currently has four fixed delivery sites and served 5,318 patients in 2023 with a full spectrum required services including primary medical, mental health, substance use disorder, and enabling services to individuals across the lifespan. Additional services are also available through formal referral arrangements. SJH is a competing continuation applicant for this Service Area Competition funding opportunity. Need to be addressed: The health care needs in the service area are complex and span access barriers and significant health disparities due to negative health behaviors and the impacts of the social determinants of health. The penetration rate of Health Center Program (HCP) among the low-income population in SJH’s service area is 22.92 percent, leaving 230,068 low-income residents not receiving care at an existing FQHC or Look-Alike. Among SJH’s patient population, 22.65 percent have hypertension according to 2023 UDS data, and 13.93 percent have diabetes. The leading causes of death in SJH’s service area are heart disease, cancer, stroke, unintentional injuries, chronic lower respiratory disease, diabetes, Alzheimer’s Disease, pneumonia and influenza, chronic liver disease and cirrhosis, hypertension, suicide, and HIV/AIDS. Other factors such as mental health issues, sexually transmitted infections, and infant mortality are also prevalent among service area residents. The negative health behaviors impacting these rates include illicit drug use, alcohol and marijuana use, risky sexual behavior, and more. The social determinants of health impacting the patient population include poverty, educational attainment, and unemployment. Population groups to be served: The service area spans 21 ZIP codes and 73.4 square miles in and around Newark, NJ. SJH’s proposed service area has a total population of 751,174. Of the 751,174 individuals, 39.7 percent (or 298,477 individuals) live below 200 percent of the FPL. Nearly 20 percent of SJH’s patients in 2023 were Public Housing patients. These residents make up the SJH’s target population, a group chronically lacking access to primary care services. In 2023, 88.52 percent of SJH’s patients with known race ethnicity identified as a racial and/or ethnic minority, 53.10 percent identified as Hispanic, Latino/a, or Spanish Origin, 37.83 percent identified as Black, 12.48 percent identified as Non-Hispanic, Latino/a, or Spanish Origin White, 0.64 percent identified as Asian, 0.38 percent identified as American Indian/Alaska Native, and 1.53 percent identified as more than one race. Total number of unduplicated patients projected to serve in 2026: SJH will serve a projected 5,419 unduplicated patients in calendar year 2026.
