Health Center Program - Central Neighborhood Health Foundation (dba as Central Neighborhood Christian Health Clinics) is a nonprofit 501(c)3 organization operating California state-licensed community clinics across three Southern California Counties (Los Angeles, Riverside, and San Bernardino). Central Neighborhood Christian Health Clinics (CNCHC) receives section 330 health center grant funds to promote and subsidize care to uninsured and under-insured, low-income populations. CNCHC’s Board of Directors and leadership team is guided by a multi-year strategic plan focused on addressing the health inequities and social determinants of health that impact the low-income target population. CNCHC is the SAC competing continuation applicant for SAAT #318. In calendar year 2023, CNCHC provided comprehensive primary care and integrated health services, including mental health, substance use disorder, and dental care to 21,067 total unduplicated patients. This represents a 32% increase in patients served compared to total patients served in 2021. CNCHC’s health services are provided from 11 fixed-site clinic locations and three mobile care clinics operating throughout the tri-county service area. Each clinic location is strategically located to be readily accessible to the identified low-income target population, located on major bus routes and within walking distance of thousands of community residents. Over the last 12 months, Cardiology, Gastroenterology, and Podiatry specialty care services have been added to CNCHC’s service mix. CNCHC serves some of the poorest and most vulnerable residents of the tri-county service area, a densely populated geographic area (150-zip codes) covering 1,335 sq. miles. The population living in CNCHC’s service area is racially and ethnically diverse, dominated by Hispanic/ Latinos (62.5% of the population) followed by, White Alone (14.6% of the population), Black/African Americans (10.6% of the population), and Asians (9.3% of the population). Over a third of the population in the service area (36.4%) report incomes below 200% federal poverty guidelines (FPG), with 15.6% of these low-income individuals living at or below the poverty level. Residents living in CNCHC’s service area are impacted by environmental, educational, nutritional, and adverse socioeconomic exposures such as violence, inadequate housing, environmental pollutant exposure, and most especially poverty. The impact of regular access to health care is vital to the grossly underserved population living in the tri-county service area. To address the health status of residents, CNCHC provides a coordinated, seamless continuity of culturally and linguistically appropriate quality care. Chronic health conditions such as diabetes, asthma, COPD, high blood pressure, obesity, coronary heart disease, anxiety, depression, and substance use are among the health issues impacting our patients. Health disparities across the tri-county service area include higher death rates for diseases of the heart, diabetes, cancers, cerebrovascular disease, Alzheimer’s disease, chronic liver disease/cirrhosis, and intentional self-harm when compared to the state’s residents. CNCHC’s system-wide approach to health care delivery is a patient-centered and integrated care model, combining the provision of primary medical care, dental, behavioral health, and social/ safety-net services accompanied by comprehensive care management. This approach addresses both the patient’s health and health-related social needs as well as establishing a collaborative approach to care, partnering patients with their care teams.
