Health Center Program - Founded in 1997, Harmony Health Medical Clinic and Family Resource Center (Harmony Health) attained FQHC status in 2015. We serve predominantly rural communities within Yuba, Sutter and Nevada Counties, California. Our mission is to serve as a community-centered provider of services and to improve the health, education, and economic opportunities for individuals and families, particularly uninsured and underinsured residents who live below 200% of the Federal Poverty Guidelines. Harmony Health cares for a diverse patient population, including Latino, Hmong, Punjabi, African American, and Native American, but most of our patients reflect a Caucasian population with the lowest income and literacy rates combined with some of the highest unemployment, substance use addiction and homelessness rates in the state, closely resembling those who live in the Appalachian Mountains. Since 2017, we have added four intermittent/satellite clinics, one (which recently combined two smaller sites) in Sutter County, a wellness center primarily for perinatal patients and children in Yuba County next to our flagship clinic, a site in Wheatland and a mobile medical unit, to allow us the ability to meet patients where they are, reducing barriers to access. Harmony Health primarily serves small communities and surrounding rural areas of Yuba, Sutter and Nevada Counties. The demographics listed above, including substance abuse, behavioral health diagnoses and treatment, lack of transportation, and misinformation, present significant barriers to obtaining needed health care and supporting services. In addition, a high percentage of service area residents lack health insurance or are covered only under Medicaid, which almost no specialists accept as payment, adding an additional burden. Harmony Health has demonstrated success in addressing pervasive health disparities and conditions in our service area by providing culturally sensitive and appropriate services designed to help break the cycle of substance abuse, chronic illness and poverty exhibited by our patient population. We combine comprehensive primary and preventative care with perinatal care, integrated behavioral health and substance abuse support services, including Medicated Assisted Treatment (MAT) for opiate use disorder, to assist individuals and families in achieving and maintaining safe, healthy, and productive lifestyles. In addition, we serve all in need, regardless of their ability to pay. have an integrated behavioral health program including a MAT program that currently includes two psychiatric nurse practitioners, a licensed clinical social worker (LCSW), a licensed marriage and family therapist (LMFT), a drug and alcohol counselor and enhanced care managers for perinatal, MAT and homeless populations of focus. Our mobile unit serves hard to reach areas and populations in the foothills, including Camptonville in Northern Yuba County and North San Juan in Nevada County, 50 miles from our main clinic to the homeless populations in both Yuba and Sutter Counties. Some ancillary services we provide include pain management through chiropractic, neurofeedback and acupuncture, insurance Outreach and Enrollment, transportation in our van to office visits, and case management for MAT, diabetic and perinatal patients. Our Street Medicine Program serves patients in the river bottoms, on the streets and at shelters, meeting them where they are. We are currently adding optometry services at our new Chestnut St., Sutter County location to address the optometric needs of our diabetic and hypertensive patients. In 2023, Harmony Health served 3,774 unique patients with 22,153 encounters, which is also Harmony Health’s proposed patient target. As the only FQHC in the region with a Street Medicine Program and a free standing birth center (a carve out) delivering babies for low risk moms in a warm, and welcoming out of hospital environment makes us quite a unique community health center.
