(HealthFirst) serves medically underserved populations in Fayette County Kentucky. Our mission is to provide quality health care to all who need us. In line with this mission, we strive for our services to be easily accessible and affordable. Our target population includes residents of the service area with incomes at or below 200% of the Federal Poverty Level (FPL) with a focus on those who are uninsured, underinsured, and persons experiencing homelessness. HFBH operates 19 sites including one administrative site and 18 service delivery sites all of which are operated by HFBG. Of the 17 fixed sites, seven are permanent and ten are seasonal. The ten seasonal sites are the school-based health centers, and the months of operation are in alignment with the school year to provide access to students and their families. By locating our sites in populated areas, we help to remove transportation barriers for our patients. In addition, SBHCs have allowed us to extend our reach into neighborhoods for children, their families, and school faculty. Children are guaranteed access to school each day and often the schools are also easily accessible to parents. Our service area population is 323,152 of whom 74.9% are White, 14.8% are African American, 3.8% are Asian, 6.0% identify as another race, and 7.2% are Hispanic/Latino. Our patient population is even more diverse with 61.7% White patients, 24.3% African American patients, 8% Asian patients, and 6% of patients identifying as another race. In addition, 32.2% of our patients are Hispanic/Latino. The service area has a poverty rate of 18.9% compared to the national rate of 15.5%. Over one third of the population in the service area (36.3%) is low- income defined as having an income below 200% FPL. While the service area has a higher rate of primary care physicians that the national rate (149.3 per 100,000 compared to 87.8 per 100,000), the population we serve still faces barriers to accessing care. More than one in five individuals (21.7%) lack a consistent source of care. Residents of our service area and particularly the patients we serve have high rates of chronic conditions including asthma (16.2%), diabetes (9.3%) and heart disease (4.8%). We have seen an alarming increase in the overdoses primarily caused by the increased use of opioids. The overdose rate is 27.0 per 100,000 compared to the national rate of 13.4 per 100,000. The population of people experiencing homelessness has increased over last year with an estimated 2,401 people experiencing homeless across all age groups. To address the need in the community, we provide preventive and acute primary care, oral health, behavioral health and pharmacy services on a sliding fee scale in accessible locations. Utilizing a total staff of 285.59 FTE, with 33.67 FTE funded with the SAC award, we anticipate serving a total of 27,000 patients in over 100,000 patient visits delivered via in-person and virtually including 20,372 medical patients, 9,803 dental patients, and 3,637 mental health patients and 2,556 enabling services patients. We will serve approximately 1,262 people experiencing homelessness, and nearly 5,000 children and families in school-based health centers.
