Housing Works Health Services III, Inc. (HWHSIII), the healthcare subsidiary of Housing Works, Inc., is licensed by the New York State Department of Health under Article 28 as a Diagnostic and Treatment Center, the NYS Office of Mental Health under Article 31 as an Outpatient Mental Health Treatment Clinic, and the NYS Office of Addiction Services and Supports under Article 32 as a Chemical Dependence Medically Supervised Outpatient Rehabilitation Center. In 1998, HWHSIII was designated as a Federally Qualified Health Center (FQHC), receiving funding for both 330(e) and 330(h) programs. HWHSIII provides integrated primary care, behavioral healthcare, and wraparound enabling and supportive services through two sites in Brooklyn: Downtown Brooklyn Health Center and East New York Community Health Center; and three sites in Manhattan: Keith D. Cylar Community Health Center, W. 37th Street Health Center, and the Park Avenue Health Center. HWHSIII also provides primary care and mental health services at the Kingsborough Menâ€™s MICA shelter in Brooklyn via contract; and provides healthcare services to individuals residing at nine temporary hotel sitesâ€”two contracted by the NYC Department of Homeless Services hotels and seven contracted by the Mayorâ€™s Office of Criminal Justice (MOCJ). HWHSIIIâ€™s proposed service area has a total population of 2,939,816 residents, of which 74.4% are people of color and 33% of residents speak a language other than English at home. In 2020, there were an estimated 3,857 unsheltered individuals in NYC (2,187 on surface area/streets and 1,670 living in the subways)â€”1,283 in Manhattan and 400 in Brooklyn, the two boroughs with the majority of HWHSIIIâ€™s in-scope ZIP codes; and approximately 50,000-60,000 individuals resided in a shelter. Many service area residents face socioeconomic hardships: 22.8% live in poverty, 5.5% are unemployed, and one in four received public assistance in the prior 12 months. Compared to New Yorkers overall, service area residents have higher rates of chronic health conditions, and these vary by neighborhood; and service area residents accounted for 62% of new HIV diagnoses in NYC in 2019. Residentsâ€™ complex health conditions are compounded by behavioral health issuesâ€”in 2018, the service areaâ€™s psychiatric and drug-related hospitalization rates (per 100,000) were 860.6 and 1,391, respectively, and both were well above NYCâ€™s rates. Through its Section 330 program, HWHSIII plans to continue to implement its comprehensive primary and behavioral healthcare and supportive services to service area residents. The integrated health centers will address the disproportionate rates of co-occurring chronic medical and behavioral health conditions and high rates of avoidable hospitalization by providing a â€œone-stop shopâ€ to address the fragmented nature of healthcare while also providing access to wraparound support services. The goal of the program is to improve community-based, proactive treatment and management of chronic physical and behavioral conditions.
