In addition to the main site, DCHC also operates four (4) local School-based Health Centers, all of which provide primary care and behavioral health services, and a dental program including a stand-alone site and mobile unit. The HSD operations include the Richland Parish Hospital (RPH), a Critical Access Hospital, and the Delhi Rural Health Clinic. Altogether the services provided under the umbrella of the Hospital Service District, include 24/7 emergency room, inpatient, outpatient (lab, physical therapy, occupational therapy, speech, radiology, mammography, and diabetes education), geriatric psychiatric services, primary care and school-based health to the residents of the Parish, and â€œgives backâ€ to the community through prescription assistance and indigent care. The DCHC service area is Richland Parish, a rural â€œDeltaâ€ parish in northeast Louisiana defined by the US Department of Agriculture as a Persistent Poverty County/Parish. RP has a significant low-income population, a substantial black population and increasing sen ior population. The parish is significantly underserved and has a host of Population Health Professional Shortage Area/Medically Underserved Area (HPSA/MUA) designations. RP has a Primary Care Low Income Single County HPSA (19), a Low-Income Single County Dental HPSA (11), a High Needs Geographic Single County Mental Health HPSA (22), and a Richland Parish MUA (59.70). The DCHC facility specific HPSA scores are Primary Care (18), Mental Health (22), and Dental Health (22). The DCHC provides a full complement of comprehensive primary health care services across the lifecycles. Services (on site and through referral) include primary care, oral health, behavioral health, substance abuse counseling, enabling services, specialty care/services, education, and comprehensive outreach. Services are offered to all residents of Richland Parish and the surrounding area regardless of language, gender, socio-economic status, sexual orientation, physical and mental capacity, age, religion, housing status, and the ability to pay. Particular attention, however, has been directed at reaching the low-income, uninsured, underinsured, Medicare/Medicaid, and vulnerable populations residing in the service area. The DCHC is forecasted to provide an underserved, low-income population of an estimated more than 6,000 patients with approximately 33,000 encounters per year. Services will be provided by a combination of physicians, midlevel clinical practitioners, dental practitioners and behavioral health providers. Per its 2022 UDS, DCHC provided care to 8,188 unduplicated patients in 32,566 visits with staffing that included 2 FTE Physicians (MD), 6.37 FTE Nurse Practitioners (NP), 12.12 FTE Behavioral Health (BH) staff, and .93 FTE Dentists. DCHC has served as the primary source of care for patients with respiratory-related illnesses, including COVID, for the community. DCHC provides services at the main site, four (4) school-based health centers, a dental clinic and a mobile dental clinic.
