MED is located in the eastern portion of Dallas, and also serves the adjacent cities of Garland, Balch Springs, Mesquite, Rockwall, Forney, Seagoville, and Royse City. MED has been serving residents in Dallas County, Texas, since 2002 when it was established by several churches after a survey showed nearly 50,000 persons in East Dallas County did not have health insurance. In 2012, MED was awarded a New Access Point, and since then, has been providing quality medical, dental, and behavioral health services to the vulnerable population in East Dallas County. MED is a nationally recognized as a patient-centered medical home. In 2018, MED was recognized nationally for its high-quality care. The target population for MED are those in the service area who have incomes at or less than 200% of the federal poverty level. Of the 844,049 residents in the service area, 41.6%, or 348,757 individuals, are in the target population. Of the target population, 69.3% are white; 23.7% are black; 4.0% are Asian; 0.5% are American Indian/Alaskan Native; and 0.0% are Hawaiian Native/Pacific Islander. In the service area, 44.6%% are of Hispanic origin. The service area has 26.4% of the population uninsured, while 17.1% has Medicaid, 0.1% has CHIP, 9.3% has Medicare, and 47.1% has private insurance. Health care needs and barriers to care. Residents in the service area have higher incidences of diabetes, obesity, and high blood pressure. In the service area, 16.1% of service area residents are living in poverty, and 41.6% have incomes below 200% of the Federal Poverty Level. MED offers all services on a sliding fee scale. Eligibility screening for publicly available health care coverage options, and assistance with enrollment, are directly provided onsite by trained Outreach/Enrollment staff. The target population has low educational attainment, resulting in literacy barriers for some of MED's patients. Addressing the need for services. Health services offered by MED include primary medical, dental care, laboratory, x-ray, and chronic disease management. Pediatric care includes well-childcare, sick child care, and immunizations. Behavioral health care includes substance abuse prevention and screening. Outreach, case management, and transportation services are provided. In addition, MED provides referrals to social services, enrollment assistance, care management, and interpretive services.
