Health Center Program - Health Center Cluster - Project Title: FY 2022 Service Area Competition (SAC) (HRSA-22-007) Applicant Organization Name: Samuel Dixon Family Health Center Website: www.sdfhc.org Grant Request: $1,627,670 Service Area Identification Number: 307 City and State: Valencia, CA Total Number of Unduplicated Patients Projected to be Served in CY 2023: 8,259 Brief Description of the Proposed Project: Samuel Dixon Family Health Centers, Inc. (SDFHC) is a Federally Qualified Health Center (FQHC) with over 40 years of experience addressing the healthcare needs of low-income, uninsured, and under-insured individuals and families in the Santa Clarita Valley in northern Los Angeles County. Within its 58-square-mile service area, SDFHC operates five in-scope sites: three full-time health centers in Newhall, Val Verde, and Canyon Country, California, as well as two outreach programs for students at College of the Canyons in Santa Clarita and California Institute of the Arts in Valencia. Needs to be Addressed: SDFHCâ€™s objective is to increase the communityâ€™s overall health, reduce suffering, and minimize impact on hospital emergency rooms and the public health system. SDFHCâ€™s service delivery approach is broad in scope, addressing the medical, oral health, behavioral health, and social complexities encountered by general community residents through a range of primary care services, chronic care/disease management, and case management. Service area residents face a range of health disparities that include high rates of cardiovascular disease, cancer, chronic lower respiratory disease, influenza/pneumonia, stroke, diabetes, Alzheimerâ€™s disease, as well as lower rates of immunization and cancer screening. Proposed Services: SDFHC offers a full range of services either directly or through referral that comprise a comprehensive primary healthcare system designed to respond to medically uninsured patients, as well as the general community. Through this SAC application, SDFHC will continue to directly provide primary care, dental, and mental health services. In addition, SDFHC will contract with providers and organizations who provide diagnostic laboratory, diagnostic radiology, gynecological and obstetrical care, dental, mental health, substance use disorder (SUD) and optometry services. SDFHC is committed to providing access to persons living at 200% or below of the Federal Poverty Guidelines (FPG) who reside in our sparsely (Val Verde site) to moderately (Newhall and Canyon Country sites) densely populated service area. Population Groups to be Served: The target population comprises service area residents whose incomes are at or below 200% of the FPG and those who are uninsured and under-insuredâ€”characteristics that currently describe a substantial proportion (43.9%) of the population. Of the service area residents living at or below 200% FPG, more than one-quarter (26.4%) are uninsured or on Medicaid. Furthermore, nearly three-quarters (72.7%) of service area residents are persons of color.
