DESCRIPTION OF PROJECT AND PROPOSED SERVICES: HCCH was created in 2010 as a 501c3 and HRSA funded in 201 3. The mission of the High Country Community Health (HCCH) team is to provide quality, patient centered, culturally appropriate, affordable and compassionate care that serves the whole person. Our purpose is to improve the health and quality of life in HCCH’s community through the provision of culturally sensitive, professional and compassionate services with special emphasis on reaching the medically underserved populations in the High Country area to include Watauga County, Avery County, Burke County, Surry County and the farmworker and Latino populations in surrounding counties. HCCH will provide (1) preventive and primary health care services in an integrated manner which will best meet individuals’ needs regardless of the ability of an individual or family to pay or their insurance status; and (2) other critical community and social services including, but not limited to, health education, community outreach, dental services, nutritional services, behavioral health, substance abuse prevention, low-barrier care programs, medication assisted therapy and treatment programs and 340B pharmacy services. In 2023, HCCH served 15,662 patients with 48,680 medical (2,610 virtual), 7,166 dental (2 virtual), 6,979 mental health (2,083 virtual), 2,024 substance use disorder (183 virtual), 949 nutrition (91 virtual), and 1,238 enabling (1 virtual) encounters (2023 UDS). In recognition of the significant unmet health care needs and the number of individuals who are not being served by the health center program, HCCH is applying for Community and Migrant Health Center Continuation of funding at $2,141,429, in order to increase access to primary care, women's health, dental, behavioral health, nutrition, migrant farmworker and health outreach needs of four counties including Watauga, Avery, Surry and Burke. NEEDS TO BE ADDRESSED: Data from the service areas reveal systemic, widespread health disparities and emphasize the need for comprehensive primary and preventive health services that are accessible to those with low incomes and the uninsured. The population to provider ratio in the service areas demonstrate a severe shortage of primary care, dental, and mental health providers, presenting a significant barrier to care, particularly for low-income residents. Increased access to mental health services in these regions is of most importance. POPULATION GROUPS TO BE SERVED: HCCH prioritizes serving low-income individuals in the service area, including Medicaid recipients and the underinsured/uninsured. HCCH is a recipient of Migrant Health Center funding and will continue to serve the MSAW (Migratory and seasonal agricultural workers) populations at our sites and in the fields through our Mobile Unit. HCCH also has an ever increasing number of patients participating in our medication assisted therapy and treatment program. HCCH’s goal is to expand our pediatric services at our main site in Boone.
